银苏治咳方治疗感染后咳嗽(风痰证)患者的临床研究

摘　　要：目的 观察银苏治咳方治疗感染后咳嗽(PIC)风痰证患者的临床疗效。方法 将300例患者随机分为西药组、中药组及联合组各100例,分别予以复方甲氧那明胶囊、银苏治咳方及复方甲氧那明胶囊联合银苏治咳方治疗,比较治疗前后的症状评分,对比临床疗效、咳嗽缓解时间及复发率。此外,每组各随机抽取30例患者,观察治疗前后的炎症指标和神经源性介质指标的变化。结果 治疗后,各组患者的中医证候积分、视觉模拟量表(VAS)评分均降低,且联合组均显著低于西药组和中药组(P <0.05);各组患者的中文版莱切斯特咳嗽问卷(LCQ-MC)评分均高于治疗前,且联合组升高更显著(P <0.05)。治疗后,各组患者血清γ干扰素(IFN-γ)水平均升高,且联合组高于西药组和中药组(P <0.05);各组患者的血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)及诱导痰P物质(SP)水平均明显下降,且联合组的上述3项指标均比西药组和中药组更低(P <0.05)。联合组的总有效率为97.00%,高于西药组的76.00%和中药组的87.00%(P <0.05)。此外,联合组的咳嗽缓解时间短于中药组和西药组(P <0.05),且复发率低于西药组与中药组(P <0.05)。结论 银苏治咳方联合复方甲氧那明胶囊治疗PIC(风痰证)能改善症状、提高生活质量、减少咳嗽缓解时间和复发率,疗效显著,其机制可能与减轻炎症反应和降低气道神经源性炎症有关。Objective:To observe the clinical efficacy of Yinsu Zhike Decoction in treating post-infectious cough with wind-phlegm syndrome.Methods:A total of 300 patients were randomly divided into three groups:western medicine group,traditional Chinese medicine(TCM)group,and combination group,100 patients in each group.The three groups were treated with compound methoxyphenamine capsules,Yinsu Zhike Decoction,and a combination of both,respectively.Symptom scores before and after treatment were compared to evaluate clinical ef-ficacy,cough relief time,and recurrence rate.Additionally,30 patients from each group were randomly selected to observe changes in inflammatory markers and neurogenic mediators before and after treatment.Results:After treatment,TCM syndrome scores and Visual Analogue Scale(VAS)scores decreased in all groups,and the combi-nation group showed significantly lower scores than both the western medicine and TCM groups(P<0.05).Leices-ter Cough Questionnaire-Chinese version(LCQ-MC)scores increased in all groups compared with before treat-ment,and the combination group showed significantly higher scores(P<0.05).Serum interferon-(IFN-)levels inereased in all groups after treatment,and the combination group had higher levels than the other two groups(P<0.05).Serum interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),and induced sputum substance P(SP)levels significantly decreased in all groups,and the combination group showed the lowest levels among the three(P<0.05).The overall effectiveness rate was highest in the combination group with 97.00%,compared with 76.00%in the western medicine group and 87.00%in the TCM group(P<0.05).Furthermore,the cough relief time was short-er in the combination group than in the other two groups(P<0.05),and it had a lower recurence rate compared with the western medicine group(P<0.05).Conclusion:Yinsu Zhike Decoction combined with compound me-thoxyphenamine capsules for post-infectious cough with wind-phlegm syndrome can improve symptoms,enhance quality of life,reduce cough relie
