疏风解毒胶囊对慢性阻塞性肺疾病急性加重期患者血清AQP-1、AQP-5、CC16含量的影响

摘　　要：目的 观察疏风解毒胶囊对慢性阻塞性肺疾病急性加重期(AECOPD)(痰热壅肺证)患者的临床效果及对血清水通道蛋白-1(AQP-1)、水通道蛋白-5(AQP-5)、人克拉拉细胞蛋白(CC16)表达水平的影响,并探寻其作用机制。方法 患者60例按照随机数字表法分为对照组与治疗组各30例。对照组予以AECOPD常规治疗(低流量氧疗、抗感染、解痉、平喘处理),治疗组在对照组常规治疗基础上加用疏风解毒胶囊,两组疗程均为1周。观察两组患者治疗前及治疗后的临床症状、中医证候积分、慢性阻塞性肺疾病评估测试(CAT)评分、肺功能指标以及血清AQP-1、AQP-5、CC16含量。结果 治疗后两组中医证候积分、CAT评分及肺功能指标均较治疗前改善(P <0.05),AQP-1、AQP-5、CC16均较治疗前改善,且治疗组改善更为显著(P <0.05)。结论 运用疏风解毒胶囊联合西医吸氧、抗感染、抗炎、解痉、平喘等治疗,能有效地改善AECOPD(痰热壅肺证)患者的临床症状、体征,降低中医证候积分、提高肺功能、降低CAT评分、改善气道炎症、提高生存质量。Objective:To observe the clinical efficacy of Shufeng Jiedu capsules in patients with AECOPD(syn⁃drome of phlegm-heat obstructing lung)and their effect on the expression levels of aquaporin-1(AQP-1),aquapo⁃rin-5(AQP-5),and human Clara cell protein(CC16)in the serum,and to explore the underlying mechanisms.Methods:60 patients were divided into a control group and a treatment group of 30 cases each according to ran⁃dom number table method.The control group received standard treatment for AECOPD(low-flow oxygen therapy,anti-infection,bronchodilation,and asthma relief).The treatment group received additional Shufeng Jiedu capsules on top of the standard treatment.Both groups underwent a one-week treatment period.Clinical symptoms,TCM syndrome scores,COPD Assessment Test(CAT)scores,and lung function indices were observed,along with chang⁃es in serum levels of AQP-1,AQP-5,and CC16.Results:After treatment,the TCM syndrome scores,CAT scores,and lung function indices improved in both groups compared with before treatment(P<0.05),and the levels of AQP-1,AQP-5,and CC16 also improved,with more significant improvements observed in the treatment group(P<0.05).Conclusion:The combination of Shufeng Jiedu capsules with western medical treatments such as oxygen therapy,anti-infection,anti-inflammatory,bronchodilation,and asthma relief can effectively improve the clinical symptoms and signs in patients with AECOPD(wind heat invading lung and phlegm-heat obstructing lung),re⁃duce TCM syndrome scores,enhance lung function,lower CAT scores,improve airway inflammation,and enhance quality of life.
