痛风舒解汤治疗急性痛风性关节炎的临床观察

摘　　要：目的 观察郭剑华经验方痛风舒解汤治疗急性痛风性关节炎(AGA)的临床疗效。方法 将60例AGA患者随机分为痛风舒解汤组与依托考昔组各30例。所有AGA患者在相同的基础治疗之上,痛风舒解汤组予以内服痛风舒解汤加减治疗,每日1剂。依托考昔组予以内服依托考昔片120 mg,每日1次。两组均连续服用8 d。比较两组治疗前后临床疗效、中医证候积分、疼痛评分、实验室指标以及治疗过程中发生不良反应的检出率。结果 痛风舒解汤组总有效率为93.33%,高于依托考昔组的80.00%(P <0.05);两组中医证候积分、疼痛评分(VAS)、血沉(ESR)、C反应蛋白(CRP)、血尿酸(BUA),与治疗前相比均有所改善;痛风舒解汤组降低BUA更明显(P <0.05);在中医证候单项积分比较中,痛风舒解汤组改善关节疼痛、局部压痛、皮色皮温、全身症状方面更明显(P <0.05或P <0.01);痛风舒解汤组不良反应检出率低于依托考昔组(P <0.05)。结论 郭剑华经验方痛风舒解汤可改善AGA患者中医证候、缓解疼痛、减轻炎症反应、减少不良反应,且能从降低BUA,改善关节疼痛、局部压痛、皮色皮温、全身症状方面提高临床疗效,体现中医治疗的独特优势。Objective:To observe the clinical efficacy of Professor Guo Jianhua′s empirical prescription,Tong⁃feng Shujie Decoction,in the treatment of acute gouty arthritis(AGA).Methods:60 AGA patients were randomly divided into the Tongfeng Shujie Decoction group(30 cases)and the etoricoxib group(30 cases).All AGA patients received the same basic treatment.The Tongfeng Shujie Decoction group was given Tongfeng Shujie Decoction,ad⁃justed according to individual conditions,once daily.The etoricoxib group was given etoricoxib tablets 120 mg once daily.Both groups were treated continuously for 8 days.The clinical efficacy,TCM syndrome scores,pain scores,laboratory indicators,and the incidence of adverse reactions during treatment were compared between the two groups.Results:The overall effective rate in the Tongfeng Shujie Decoction group(93.33%)was higher than that in the etoricoxib group(80.00%)(P<0.05).Both groups showed improvements in TCM syndrome scores,pain scores(VAS),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),and blood uric acid(BUA)com⁃pared with before treatment;the Tongfeng Shujie Decoction group showed a more significant reduction in BUA(P<0.05).In the comparison of single-item TCM syndrome scores,the Tongfeng Shujie Decoction group showed more significant improvements in joint pain,local tenderness,skin color and temperature,and systemic symptoms(P<0.05 or P<0.01).The incidence of adverse reactions in the Tongfeng Shujie Decoction group was lower than that in the etoricoxib group(P<0.05).Conclusion:Professor Guo Jianhua′s empirical prescription,Tongfeng Shujie Decoction,can improve TCM syndromes,relieve pain,reduce inflammatory reactions,and decrease adverse reac⁃tions.It can also enhance clinical efficacy by lowering blood uric acid and improving joint pain,local tenderness,skin color and temperature,and systemic symptoms,highlighting the unique advantages of TCM treatment.
