益气通腑方灌肠治疗脓毒症肠功能障碍的临床观察

摘　　要：目的 观察益气通腑方灌肠治疗脓毒症肠功能障碍的疗效。方法 60例脓毒症肠功能障碍患者采用随机数字表法分为两组各30例,对照组患者采用西医常规治疗及开塞露灌肠,治疗组患者在西医常规治疗基础上采用益气通腑方灌肠治疗,比较两组疗效。结果 治疗后,治疗组总有效率为90.00%,高于对照组的66.67%(P <0.05);治疗组患者胃肠功能障碍分级(GDS)评分、急性生理与慢性健康量表(APACHEⅡ)评分低于对照组,腹腔压力、胃残余量小于对照组,每分钟肠鸣音次数多于对照组;血清D-乳酸、肠脂肪酸结合蛋白(IFABP)、二胺氧化酶(DAO)、降钙素原(PCT)、肿瘤坏死因子-α(TNF-ɑ)、白细胞介素-6(IL-6)水平低于对照组,ICU住院时间短于对照组(均P <0.05)。结论 益气通腑方灌肠能够提高脓毒症肠功能障碍患者疗效,改善患者胃肠功能,减轻炎症反应,缩短住院时间。Objective:To observe the fficacy of Yiqi Tongfu Decoction enema in treating sepsis-associated in-testinal dysfunction.Methods:60 patients with sepsis-associated intestinal dysfunction were randomly divided in-to two groups using a random number table method,30 cases in each group.The control group received convention-al western medical treatment and glycerin enema,while the treatment group received Yiqi Tongfu Decoction enema on top of conventional western medical treatment.The efficacies of both groups were compared.Results:After treatment,the total effective rate in the treatment group was 90.00%,which was higher than 66.67%in the control group(P<0.05).The treatment group showed lower gastrointestinal dysfunetion scores(GDS),Acute Physiology and Chronic Health Evaluation I(APACHE I)scores,intra-abdominal pressure,gastric residual volume,and higher bowel sounds per minute compared with the control group(P<0.05).Serum levels of D-lactic acid,intesti-nal fatty acid-binding protein(IFABP),diamine oxidase(DAO),procalcitonin(PCT),tumor necrosis factor-alpha(TNF-α),and interleukin-6(IL-6)were lower in the treatment group,and ICU stay duration was shorter com-pared with the control group(P<0.05).Conclusion:Yiqi Tongfu Decoction enema can improve the efficacy in pa-tients with sepsis-associated intestinal dysfunction,enhance gastrointestinal function,reduce inflammatory respons-es,and shorten hospital stays.
