血清CRP、SAA、IL-6水平联合检测对甲型H1N1流感的诊断价值

摘　　要：目的:探究甲型H1N1流感患者血清C-反应蛋白(CRP)、淀粉酶A蛋白(SAA)、白细胞介素-6(IL-6)水平并分析其对甲型H1N1流感的诊断价值。方法:选取2020年6月—2023年6月某院收治的102例甲型H1N1流感患儿为甲流组,乙型流感病毒患儿102例为乙流组,另选取同期健康志愿者34例为对照组。比较3组血清CRP、SAA、IL-6水平及实验室指标,依据病情严重程度将甲流组分为轻症者76例、重症者26例,比较其血清CRP、SAA、IL-6水平,分析血清CRP、SAA、IL-6水平与实验室指标、病情严重程度相关性,采用受试者工作特征曲线(ROC)分析各血清指标对甲型H1N1流感的诊断价值。结果:甲流组血清CRP、SAA、IL-6水平分别为(5.62±1.47)mg/L、(43.26±11.42)mg/L、(15.96±4.32)ng/mL,高于乙流组的(4.11±1.30)mg/L、(36.57±11.19)mg/L、(12.24±3.08)ng/mL和对照组的(2.18±0.70)mg/L、(12.52±4.08)mg/L、(6.85±2.18)ng/mL,差异有统计学意义(P<0.05)。重症者血清CRP、SAA、IL-6水平分别为(8.16±2.52)mg/L、(52.32±11.44)mg/L、(24.55±6.18)ng/mL,高于轻症者的(4.75±1.38)mg/L、(40.16±10.38)mg/L、(13.02±3.34)ng/mL,差异有统计学意义(P<0.05)。血清CRP、SAA、IL-6水平与中性粒细胞百分比呈负相关,与单核细胞百分比、病情严重程度呈正相关(P<0.05);血清CRP、SAA、IL-6水平单项及联合诊断甲型H1N1流感的AUC值分别为0.761、0.746、0.780、0.932。结论:甲型H1N1流感患儿血清CRP、SAA、IL-6水平升高,且与病情严重程度呈正相关,联合检测对甲型H1N1流感具有一定诊断价值。Objective:To investigate the levels of serum C-reactive protein(CRP),amylase A protein(SAA),and interleukin-6(IL-6)in patients with influenza A H1N1 and to analyze their diagnostic value for influenza A H1N1.Methods:A total of 102 children with influenza A H1N1 admitted to a hospital from June 2020 to June 2023 were selected as the influenza A group,and 102 children with influenza B virus were selected as the influenza B group.In addition,34 healthy volunteers were selected as the control group.The levels of serum CRP,SAA,IL-6 and laboratory indicators were compared among the three groups.The influenza A group was divided into a mild subgroup(76 cases)and a severe subgroup(26 cases)according to the severity of the condition,and their serum CRP,SAA,and IL-6 levels were compared.The correlation of serum CRP,SAA,IL-6 levels with laboratory indicators and severity of the disease were analyzed.Receiver operating characteristic curve(ROC)was used to analyze the diagnostic value of various serum indicators for influenza A H1N1.Results:The levels of serum CRP,SAA,and IL-6 in the influenza A group were(5.62±1.47)mg/L,(43.26±11.42)mg/L,and(15.96±4.32)ng/mL,respectively,higher than those in the influenza B group(4.11±1.30)mg/L,(36.57±11.19)mg/L,and(12.24±3.08)ng/mL,as well as the control group(2.18±0.70)mg/L,(12.52±4.08)mg/L,and(6.85±2.18)ng/mL,with statistically significant differences(P<0.05).The levels of serum CRP,SAA,and IL-6 in the severe cases were(8.16±2.52)mg/L,(52.32±11.44)mg/L,and(24.55±6.18)ng/mL,respectively,higher than those in the mild cases(4.75±1.38)mg/L,(40.16±10.38)mg/L,and(13.02±3.34)ng/mL,with statistically significant differences(P<0.05).The levels of serum CRP,SAA,and IL-6 were negatively correlated with the percentage of neutrophils,and positively correlated with the percentage of monocytes and the severity of the disease(P<0.05).The AUC values of serum CRP,SAA,IL-6 levels for single and combined diagnosis of influenza A H1N1 were 0.761,0.746,0.780,and 0.932,respectively.Conclusion:The
