摘 要:目的:基于IL-23/Th17免疫炎症轴探讨长蛇灸干预强直性脊柱炎(AS)小鼠的作用机制研究。方法:清洁级雌性健康BALBC/c小鼠随机分为空白组、模型组、长蛇灸组与赛来西布组,每组8只。采用牛源软骨蛋白聚糖加DDA佐剂制成混悬液腹腔注射构建AS小鼠模型。长蛇灸组予长蛇灸督脉经穴(大椎穴至腰俞穴)与两侧华佗夹脊穴(颈1至骶髂关节部)治疗,2次/周(每周三、周六施灸),灸3壮/次,连续治疗1个月(共灸8次),赛来西布组予赛来西布13 mg/kg/d灌胃治疗,1次/d,连续治疗1个月。治疗结束后,采用酶联免疫吸附试验(ELISA)法检测外周血白细胞介素-17(IL-17)、白细胞介素-23(IL-23)及肿瘤坏死因子(TNF-α);脾脏指数SI、胸腺指数TI检测;Western blot法测量回肠组织IL-17、IL-23蛋白表达水平;苏木素-伊红染色(HE)对后爪、骶髂关节进行组织病理学检测。结果:与空白组比较,模型组小鼠外周血IL-17、IL-23及TNF-α明显升高(P<0.05),回肠组织IL-17、IL-23蛋白表达水平增高,HE染色后爪、骶髂关节组织炎症细胞浸润明显增多,提示造模成功。干预结束后,与模型组比较,长蛇灸组与赛来西布组TNF-α、IL-17、IL-23、SI和TI均明显降低(P<0.05),长蛇灸组与赛来西布组比较,TNF-α、IL-17、IL-23、SI和TI差异无统计学意义(P>0.05)。与模型组比较,长蛇灸组与赛来西布组回肠组织IL-17、IL-23蛋白表达水平相对较低,且长蛇灸组蛋白表达水平降低最显著;与模型组比较,长蛇灸组与赛来西布组后爪、骶髂关节变形不规则的淋巴细胞浸润均减少,骶髂关节面边缘不整齐(毛糙),关节纤维软骨细胞增生,而长蛇灸组较赛来西布组变形不规则的淋巴细胞浸润减少更明显。结论:AS动物模型(PGIA小鼠模型)构建成功,经蛋白聚糖(PG)小鼠模型体内存在IL-23/Th17免疫炎症轴失衡,长蛇灸可通过调控IL-23/Th17轴发挥免疫调节、抗炎的作用,并且免疫调节与抗炎�Objective:To investigate the action mechanism of long snake moxibustion intervention in mice with ankylosing spondylitis(AS)based on IL-23/Th17 immunoinflammatory axis.Methods:Clean female healthy BALBC/c mice were randomly divided into the blank group,the model group,the long snake moxibustion group and the Diclofenac Sodium group,with 8 mice in each group.The mice model of AS was established by intraperitoneal injection of Bovine Chondroproteoglycan and DDA adjuvant.The long snake moxibustion group was treated with long snake moxibustion applied to points from Dazhui(DU14)to Yaoshu(DU2)of Du meridian,as well as to Huatuo Jiaji points on both sides from C1 to sacroiliac joint region,twice a week for a month,three rounds of moxibustion each time(8 rounds in total).The Diclofenac Sodium group was treated with Diclofenac Sodium suspension,13 mg/kg/d by gavage,once per day for a month.After the treatment,the levels of interleukin-17(IL-17),interleukin-23(IL-23)and tumor necrosis factor(TNF-α)in peripheral blood were detected by ELISA.Spleen index(SI)and thymus index(TI)were calculated.The protein expressions of IL-17 and IL-23 in ileum tissues were measured by Western blot.HE was used for histopathological examination of the posterior claw and sacroiliac joints.Results:Compared with those in the blank group,the levels of IL-17,IL-23 and TNF-αin peripheral blood were significantly increased(P<0.05),the protein expressions of IL-17 and IL-23 in ileum tissues were increased(P<0.05),the inflammatory cell infiltration in the tissues of posterior claw and sacroiliac joint was significantly increased in the model group,indicating successful modeling.After the interventions,the levels of TNF-α,IL-17 and IL-23 in peripheral blood,as well as the values of SI and TI were significantly decreased in the long snake moxibustion group and the Diclofenac Sodium group compared with those in the model group(P<0.05),in which there were no statistical differences between the long snake moxibustion group and the Diclofenac Sodium grou
