基于大数据挖掘肝-脾-肾同治法治疗腰痛病的临床实践及理论探索

摘　　要：目的将云南省保山市第二人民医院信息系统中中药复方数据挖掘整理后导入古今医案云平台,探讨中医“肝-脾-肾”同治思路在腰痛病治疗中的可行性。方法以古今医案云平台、药智数据、万方数据库、PubMed、Embase、CNKI、Web of Science数据库中2016-2022年中药复方治疗腰痛病相关文献为参考条件,结合云南省保山市第二人民医院信息系统中近5年中医复方治疗腰痛的中药处方建立Microsoft Excel数据集,对近年来使用的中药复方的四气、五味、归经进行分析,对高频次出现的中药进行整体性探索。结果共获得7471个中药复方,复方中中药四气主要集中在温、平、寒;五味主要集中在苦、辛、甘;归经主要集中在肝、脾、肾;对频率2000次以上的13个中药进行分析,可知四气五味集中在温苦,归经集中在“肝、脾、肾”经。结论从古至今,中药复方运用中四气“温”性行气止痛、散寒止痛、温经止痛特性,体现了腰痛病“肝-脾-肾”同治思路目前仍是临床中最为常用的治疗方法,后续可结合现代医学手段,在临床上进一步挖掘其真正靶点,为治疗腰痛病寻求更为科学、有效的中药复方。Objective To import the data mining of traditional Chinese medicine compound in the information system of Baoshan Second People's Hospital of Yunnan Province into the cloud platform of ancient and modern medical records,and to explore the feasibility of the idea of“liver-spleen-kidney”in the treatment of low back pain.Methods Based on the literature related to the treatment of low back pain by traditional Chinese medicine compound in the database of ancient and modern medical case cloud platform,Yaozhi data,Wanfang database,PubMed,Embase,CNKI and Web of Science from 2016 to 2022,Microsoft Excel data set was established in combination with the traditional Chinese medicine prescription of traditional Chinese medicine compound for the treatment of low back pain in the information system of Baoshan Second People's Hospital of Yunnan Province in the past five years.The four qi,five flavors and meridian tropism of traditional Chinese medicine compound used in recent years were analyzed,and the traditional Chinese medicine with high frequency was explored as a whole.Results A total of 7471 traditional Chinese medicine compound prescriptions were obtained.The four qi of traditional Chinese medicine in the compound prescriptions were mainly concentrated in warm,flat and cold.The five flavors were mainly concentrated in bitter,pungent and sweet;meridian mainly concentrated in the liver,spleen,kidney;by analyzing 13 traditional Chinese medicines with a frequency of more than 2000 times,it could be seen that the four qi and five flavors were concentrated in warm and bitter,and the meridian tropism is concentrated in the“liver,spleen and kidney”meridians.Conclusion From ancient times to the present,the application of traditional Chinese medicine compound has the characteristics of four qi“warm”in promoting qi and relieving pain,dispelling cold and relieving pain,warming meridians and relieving pain,which reflects that the idea of“liver-spleen-kidney”simultaneous treatment of low back pain is still the most com
