摘　　要：目的研究上海市黄浦区40岁以上参与宫颈癌和乳腺癌筛查(简称“两癌筛查”)女性健康相关生命质量及影响因素。方法采用横断面研究,随机整群抽取上海市黄浦区6个社区658名40岁及以上“两癌筛查”女性居民进行调查,基于欧洲五维三水平健康量表(EQ-5D-3L)评价健康相关生命质量。采用多因素分位数回归和Tobit回归模型分析健康相关生命质量影响因素。结果614名研究对象在EQ-5D 5个维度中疼痛或不适问题最常见(12.70%),VAS评分为86.41分,健康效用值为0.979。VAS评分多因素分位数回归分析结果显示,初中毕业、已婚、家庭人均月收入≥5000元者健康相关生命质量较高(P<0.05),生活或工作压力较大、存在慢病史者健康相关生命质量较低(P<0.05)。健康效用值Tobit回归分析显示,超重或肥胖、生活或工作压力较大者健康相关生命质量较差(P<0.05)。结论上海市黄浦区40岁及以上“两癌筛查”女性健康相关生命质量较好,文化程度、婚姻状况、经济收入、慢病史、超重或肥胖及慢病史是健康相关生命质量的影响因素。Objective To study the health-related quality of life and its influencing factors among women aged over 40 years from two cancers(breast and cervical cancer)screen program in Huangpu District,Shanghai.Methods A cross-sectional study was conducted among 658 female residents aged 40 years and above in six communities in Huangpu District of Shanghai by random cluster sampling.The health-related quality of life was evaluated based on the European five-dimensional and three-level health scale(EQ-5D-3L).Multivariate quantile regression and Tobit regression model were used to analyze the influencing factors of health-related quality of life.Results Among 614 study participants,pain/discomfort problems were the most common(12.70%)in the five dimensions of the EQ-5D,with an average VAS score of 86.41 and a mean health utility index of 0.979.The results of multivariate quantile regression analysis of VAS score showed that the health-related quality of life was higher in those who graduated from junior high school,married,and family per capita monthly income≥5000 yuan(P<0.05),and the health-related quality of life was lower in those who lived or worked under greater pressure and had a chronic medical history(P<0.05).Tobit regression analysis of health utility value showed that overweight or obesity,life or work stress were associated with poor health-related quality of life(P<0.05).Conclusion The health-related quality of life of women aged 40 and above in Huangpu District of Shanghai is good.Education level,marital status,economic income,chronic disease history,overweight or obesity and chronic disease history are the influencing factors of health-related quality of life.
