参芪地黄汤化裁方治疗早期糖尿病肾病的临床疗效及对其中医症状积分的影响

摘　　要：目的参芪地黄汤化裁方治疗早期糖尿病肾病的临床疗效及对其中医症状积分的影响。方法选取2022年5月-2023年5月在我院诊治的72例早期糖尿病肾病患者为研究对象,采用随机数字表法分为对照组和观察组,各组36例。对照组采用常规西药治疗,观察组在对照组基础上给予参芪地黄汤化裁方治疗,比较两组临床疗效、中医症状积分、血糖指标、肾功能指标。结果观察组治疗总有效率为94.44%,高于对照组的83.33%(P<0.05);两组中医症状积分均低于治疗前,且观察组低于对照组(P<0.05);两组组空腹血糖(FPG)、餐后2 h血糖(2 h PBG)、糖化血红蛋白(HbA1c)均低于治疗前,且观察组均低于对照组(P<0.05);两组治疗后血肌酐(Scr)、尿素氮(BUN)与治疗前比较,及组间比较,差异均无统计学意义(P>0.05);两组治疗后血尿酸(UA)低于治疗前(P<0.05),但组间比较,差异无统计学意义(P>0.05)。结论参芪地黄汤化裁方治疗早期糖尿病肾病具有确切的效果,可提高治疗总有效率,改善血糖水平,减轻中医症状,控制早期糖尿病肾病进展,保护肾功能,值得临床加以应用。Objective To investigate the clinical efficacy of modified Shenqi Dihuang decoction in the treatment of early diabetic nephropathy and its effect on TCM symptom scores.Methods A total of 72 patients with early diabetic nephropathy diagnosed and treated in our hospital from May 2022 to May 2023 were selected as the research objects.They were divided into control group and observation group by random number table method,with 36 patients in each group.The control group was treated with conventional western medicine,and the observation group was treated with modified Shenqi Dihuang decoction on the basis of the control group.The clinical efficacy,TCM symptom score,blood glucose index and renal function index were compared between the two groups.Results The total effective rate of treatment in the observation group was 94.44%,which was higher than 83.33%in the control group(P<0.05).The TCM symptom score of the two groups was lower than that before treatment,and that of the observation group was lower than that of the control group(P<0.05).The fasting blood glucose(FPG),2 h postprandial blood glucose(2 h PBG)and glycosylated hemoglobin(HbA1c)in the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group(P<0.05).There were no significant difference in serum creatinine(Scr)and urea nitrogen(BUN)between the two groups after treatment and before treatment,and there were no significant difference between the two groups(P>0.05).The serum uric acid(UA)of the two groups after treatment was lower than that before treatment(P<0.05),but there was no significant difference between the two groups(P>0.05).Conclusion Modified Shenqi Dihuang decoction has a definite effect in the treatment of early diabetic nephropathy,which can improve the total effective rate of treatment,improve blood glucose level,reduce TCM symptoms,control the progress of early diabetic nephropathy,and protect renal function.It is worthy of clinical application.
