摘  要:目的:探究一氧化氮吸入(iNO)配合高频振荡通气(HFOV)治疗对新生儿持续肺动脉高压(PPHN)患儿的影响。方法:回顾性分析2020年1月-2023年12月九江市妇幼保健院收治的80例PPHN患儿的临床资料,按照不同治疗方式分两组,各40例。对照组接受HFOV治疗,观察组接受iNO配合HFOV治疗。比较两组临床疗效、血气指标、肺动脉参数、左心功能及并发症发生情况。结果:观察组治疗总有效率较对照组高,差异有统计学意义(P<0.05)。治疗前,两组血气指标、肺动脉参数、左心功能比较,差异均无统计学意义(P>0.05);治疗后,观察组动脉二氧化碳分压(PaCO_(2))、肺动脉内径、肺动脉压力(PAP)、心肌做功指数(Tei指数)均较对照组低,动脉氧分压(PaO_(2))、血氧饱和度(SaO_(2))、左心室收缩期峰值速度、左心室二尖瓣环舒张早期峰值速度、左心室输出量均较对照组高,差异均有统计学意义(P<0.05)。观察组并发症发生率较对照组低,差异有统计学意义(P<0.05)。结论:iNO配合HFOV治疗PPHN患儿效果显著,可调节血气指标及肺动脉参数,改善左心功能,减少并发症。Objective:To explore the effect of nitric oxide inhalation(iNO)combined with high frequency oscillatory ventilation(HFOV)on neonates with persistent pulmonary hypertension(PPHN).Method:Clinical data of 80 children with PPHN admitted to Jiujiang Maternal and Child Health Hospital from January 2020 to December 2023 were retrospectively analyzed,and they were divided into two groups according to different treatment methods,with 40 cases in each group.The control group received HFOV treatment,and the observation group received iNO combined with HFOV treatment.The clinical efficacy,blood gas indexes,pulmonary artery parameters,left heart function and complications were compared between the two groups.Result:The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in blood gas indexes,pulmonary artery parameters and left heart function between the two groups(P>0.05).After treatment,the arterial partial pressure of carbon dioxide(PaCO_(2)),pulmonary artery diameter,pulmonary artery pressure(PAP)and myocardial work index(Tei index)in observation group were lower than those in control group,arterial oxygen partial pressure(PaO_(2)),blood oxygen saturation(SaO_(2)),left ventricular systolic peak velocity,left ventricular mitral ring diastolic peak velocity and left ventricular output were all higher than those of control group,the differences were statistically significant(P<0.05).The complication rate of the observation group was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion:iNO combined with HFOV is effective in the treatment of PPHN children,it can adjust blood gas indexes and pulmonary artery parameters,improve left heart function and reduce complications.
