经会阴和耻骨联合部超声治疗在Ⅲ型前列腺炎中的临床效果

摘　　要：目的:探讨经会阴和耻骨联合部超声治疗在Ⅲ型前列腺炎中的临床效果。方法:选取2023年3月-2024年5月岳阳市人民医院收治的Ⅲ型前列腺炎患者168例,按照随机数字表法分为两组。对照组(n=84)采用常规药物治疗,观察组(n=84)在对照组基础上采用经会阴和耻骨联合部超声治疗。比较两组临床疗效、慢性前列腺炎症状指数(CPSI)、尿流动力学指标[最大尿流率(MFR)、平均尿流率(AFR)]及前列腺液指标[卵磷脂小体(SPL)、白细胞(WBC)]。结果:观察组总有效率(92.86%)高于对照组(82.14%),差异有统计学意义(P<0.05)。治疗后,观察组CPSI各项评分及WBC水平均低于对照组,MFR、AFR、SPL水平均高于对照组,差异均有统计学意义(P<0.05)。结论:经会阴和耻骨联合部超声治疗Ⅲ型前列腺炎的效果确切,能有效改善患者前列腺症状及尿流率,抑制前列腺炎症反应。Objective:To explore the clinical effect of transperineal and pubic symphysis ultrasound therapy in categoryⅢprostatitis.Method:A total of 168 patients with categoryⅢprostatitis treated in Yueyang People's Hospital from March 2023 to May 2024 were selected and divided into two groups according to random number table method.The control group(n=84)was treated with conventional drugs and the observation group(n=84)was treated with transperineal and pubic symphysis ultrasound therapy on the basis of the control group.The clinical efficacy,chronic prostatitis symptom index(CPSI),urodynamics indexes[maximum flow rate(MFR),average flow rate(AFR)]and expressed prostatic secretion indexes[small particle of lecithin(SPL),white blood cells(WBC)]were compared between the two groups.Result:The total effective rate of the observation group(92.86%)was higher than that of the control group(82.14%),the difference was statistically significant(P<0.05).After treatment,CPSI scores and WBC levels in the observation group were lower than those in the control group,MFR,AFR and SPL levels were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:The application of transperineal and pubic symphysis ultrasound therapy in categoryⅢprostatitis has a definite effect,can effectively improve prostate symptoms and urinary flow rate,and inhibit prostatic inflammatory response.
