摘　　要：目的:探究收缩期峰值流速(PSV)在糖尿病下肢动脉硬化闭塞症患者经皮血管腔内成形术后的应用。方法:收集2022年1月-2023年5月南昌大学第四附属医院内分泌科收治的行单纯球囊扩张术治疗的80例糖尿病下肢动脉硬化闭塞症患者的临床资料。术前、术后1周及6个月检测患者患肢股浅动脉PSV及踝肱指数(ABI)、趾肱指数(TBI);采用Pearson相关系数分析PSV和ABI、TBI的相关性。结果:术后1周、6个月,糖尿病下肢动脉硬化闭塞症患者患肢股浅动脉PSV均低于术前,TBI、ABI水平均高于术前,差异均有统计学意义(P<0.05);术后6个月,糖尿病下肢动脉硬化闭塞症患者的PSV、TBI、ABI水平与术后1周比较,差异均无统计学意义(P>0.05)。Pearson相关性分析显示,PSV与ABI、TBI均呈负相关(P<0.05)。结论:PSV可作为评估糖尿病下肢动脉硬化闭塞症患者经皮血管腔内成形术后疗效的指标,在指导经皮血管腔内成形术方面起到可靠的效果,且无创、安全,是评估介入手术治疗预后的优选临床方法。Objective:To investigate the application of peak systolic velocity(PSV)in patients with diabetic lower extremity arteriosclerosis obliterans after percutaneous endovascular angioplasty.Method:Clinical data of 80 patients with diabetic lower extremity arteriosclerosis obliterans who were treated by balloon dilatation alone in the Department of Endocrinology of the Fourth Affiliated Hospital of Nanchang University from January 2022 to May 2023 were collected.Superficial femoral artery PSV of affected limb,ankle-brachial index(ABI),toe-brachial index(TBI)were detected before surgery,1 week and 6 months after surgery.Pearson correlation coefficient was used to analyze the correlation between PSV,ABI and TBI.Result:At 1 week and 6 months after surgery,PSV of superficial femoral artery in the affected limb of diabetic lower extremity arteriosclerosis obliterans patients was lower than that before surgery,and TBI and ABI levels were higher than those before surgery,the differences were statistically significant(P<0.05).At 6 months after surgery,there were no significant differences in PSV,TBI and ABI levels in patients with diabetic lower extremity arteriosclerosis obliterans compared with 1 week after surgery(P>0.05).Pearson correlation analysis showed that PSV was negative correlated with ABI and TBI(P<0.05).Conclusion:PSV can be used as an indicator to evaluate the curative effect of percutaneous endovascular angioplasty in patients with diabetic lower extremity arteriosclerosis obliterans,and it has a reliable effect in guiding percutaneous endovascular angioplasty,is non-invasive and safe,and is the preferred clinical method to evaluate the prognosis of interventional surgery.
