羟考酮注射液联合舒芬太尼注射液在妇科腔镜手术后的镇痛效果

摘　　要：目的:探究羟考酮注射液联合舒芬太尼注射液在妇科腔镜手术后镇痛中的应用价值。方法:回顾性分析2023年7月—2024年6月抚州市东乡区人民医院收治的60例妇科腔镜手术患者,按术后镇痛方案差异分为对照组、观察组,各30例。对照组在常规用药基础上加用舒芬太尼注射液,观察组采用羟考酮注射液联合舒芬太尼注射液镇痛。比较两组内脏疼痛程度、镇静状况、镇痛泵有效按压次数、补救镇痛比例及不良反应发生率。结果:术后12、24、48 h,观察组视觉模拟评分法(VAS)评分均低于对照组,Ramsay镇静评分均高于对照组,镇痛泵有效按压次数、补救镇痛比例均少于对照组,差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:与舒芬太尼注射液单独应用比较,羟考酮注射液联合舒芬太尼注射液在妇科腔镜手术后可提供较好的镇痛、镇静效果,镇痛泵有效按压次数较少,不良反应发生率较低。Objective:To explore the application value of Hydromorphone injection combined with Sufentanil injection in analgesia after gynecological endoscopic surgery.Method:A retrospective analysis was performed on 60 gynecological endoscopic surgery patients admitted to Dongxiang District People's Hospital of Fuzhou City from July 2023 to June 2024,and they were divided into control group and observation group according to postoperative analgesia regimen,with 30 cases in each group.The control group was given Sufentanil injection on the basis of conventional drugs,and the observation group was given Hydromorphone injection combined with Sufentanil injection for analgesia.The degree of visceral pain,sedation,the number of effective analgesic pump,the proportion of remedial analgesia and the incidence of adverse reactions were compared between the two groups.Result:At 12,24 and 48 h after surgery,the visual analogue scale(VAS)scores of the observation group were lower than those of the control group,the Ramsay sedation scores were higher than those of the control group,the number of effective analgesic pump compressions and the proportion of remedial analgesia cases were lower than those of the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Compared with Sufentanil injection alone,Hydromorphone injection combined with Sufentanil injection can provide better analgesic and sedative effects after gynecological endoscopic surgery,with fewer effective times of analgesic pump and a lower incidence of adverse reactions.
