摘 要:目的:探讨不同产次与产后盆底肌力、肌电值变化的关系。方法:选择2022年1月—2023年12月江西省妇幼保健院102例产后6周第一次复查的产妇为研究对象,将初次分娩的产妇纳入初产妇组(n=52),其余非初产的产妇纳入经产妇组(n=50)。比较两组各时段(产前、产后6周、产后3个月、产后6个月)盆底肌力情况和产后各时段(产后6周、产后3个月、产后6个月)盆底表面肌电值变化情况。结果:初产妇组产后3个月、产后6个月的快速收缩阶段最大值、紧张收缩阶段平均值、耐力收缩阶段平均值均大于产后6周和经产妇组(P<0.05);初产妇组产后6个月的快速收缩阶段最大值、紧张收缩阶段平均值、耐力收缩阶段平均值均大于产后3个月(P<0.05)。初产妇组与经产妇组各时段Ⅰ类肌力下降率、Ⅱ类肌力下降率对比差异均无统计学意义(P>0.05)。结论:与初产妇相比,经产妇对产后盆底肌电值的影响较大,但不同产次对产后盆底肌力的影响不大。Objective:To investigate the relationship between different births and changes in postpartum pelvic floor muscle strength and electromyographic values.Method:From January 2022 to December 2023,102 pregnant women who were reviewed for the first 6 weeks postpartum in Jiangxi Maternal and Child Health Hospital were selected as the research objects.The women who gave birth for the first time were included in the primiparous maternal group(n=52),and the rest of the women who did not give birth for the first time were included in the transient group(n=50).Pelvic floor muscle strength at each time period(prenatal,6 weeks postnatal,3 months postnatal and 6 months postnatal)and changes in pelvic floor surface electromyographic values at each time period(6 weeks postnatal,3 months postnatal and 6 months postnatal)between the two groups were compared.Result:The maximum value during rapid contraction phase,average value during tension contraction phase,average value during endurance contraction phase at 3 months postpartum and 6 months postpartum in the primiparous maternal group were higher than those in the 6 weeks postpartum and the transient group(P<0.05),while the maximum value during rapid contraction phase,average value during tension contraction phase,average value during endurance contraction phase at 6 months postpartum in the primiparous maternal group were significantly higher than those in the 3 months postpartum(P<0.05).There were no statistically significant differences in the comparison of the rates of decline in classⅠmuscle strength and classⅡmuscle strength between the primiparous maternal group and the transient group at each time period(P>0.05).Conclusion:Compared with the primiparous women,the postpartum women have a greater influence on the postpartum pelvic floor electromyographic value,but different parities have little effect on the postpartum pelvic floor muscle strength.
