摘　　要：目的:分析白细胞介素-6(IL-6)、降钙素原(PCT)、C反应蛋白(CRP)、脑钠肽(BNP)水平评估重症感染患者血流感染的价值。方法:回顾性分析2020年1月-2023年12月景德镇市第一人民医院收治的150例重症感染患者的临床资料。所有患者均进行血培养,并根据血培养结果分为阳性组、阴性组,各75例。比较两组血清IL-6、PCT、CRP、BNP水平,使用受试者操作特征(ROC)曲线评估血清IL-6、PCT、CRP、BNP水平单独及联合评估重症感染患者血流感染的价值。结果:阳性组血清IL-6、PCT、CRP、BNP水平均显著高于阴性组,差异均有统计学意义(P<0.05)。ROC曲线显示,血清IL-6、PCT、CRP、BNP水平评估重症感染患者血流感染的曲线下面积(AUC)分别为0.734、0.797、0.694、0.698,敏感度分别为62.67%、65.33%、62.67%、85.33%,特异度分别为80.00%、85.33%、74.67%、49.33%;血清IL-6、PCT、CRP、BNP水平联合评估重症感染患者血流感染的AUC为0.914,敏感度为88.00%,特异度为80.00%。结论:血清IL-6、PCT、CRP、BNP水平联合评估重症感染患者血流感染情况有较高价值。Objective:To analyze the levels of interleukin-6(IL-6),procalcitonin(PCT),C reactive protein(CRP)and brain natriuretic peptide(BNP)in the evaluation of bloodstream infection in patients with severe infection.Method:The clinical data of 150 patients with severe infection admitted to the First People's Hospital of Jingdezhen from January 2020 to December 2023 were retrospectively analyzed.All patients underwent blood culture and were divided into positive group and negative group according to the blood culture results,with 75 cases in each group.The levels of serum IL-6,PCT,CRP and BNP were compared between the two groups,and the value of serum IL-6,PCT,CRP and BNP levels alone and in combination in the evaluation of bloodstream infection in patients with severe infection were evaluated by receiver operating characteristic(ROC)curve.Result:The levels of serum IL-6,PCT,CRP and BNP in the positive group were significantly higher than those in the negative group,the differences were statistically significant(P<0.05).The ROC curve showed that the area under the curve(AUC)of serum IL-6,PCT,CRP and BNP levels for evaluating bloodstream infection in patients with severe infection were 0.734,0.797,0.694 and 0.698,respectively,the sensitivities were 62.67%,65.33%,62.67%and 85.33%,respectively,and the specificities were 80.00%,85.33%,74.67%and 49.33%,respectively,the AUC,sensitivity,and specificity of the combined assessment of serum IL-6,PCT,CRP and BNP levels for bloodstream infection in patients with severe infection were 0.914,88.00%and 80.00%,respectively.Conclusion:The combination of serum IL-6,PCT,CRP and BNP levels has a high value in the evaluation of bloodstream infection in patients with severe infection.
