个案管理模式在老年吸入性肺炎共病患者中的应用

摘　　要：总结1例高龄老年吸入性肺炎共病患者的护理个案。根据患者情况采取动态老年综合评估,制定科学个性化的护理方案,措施具体包括老年吸入性肺炎住院患者的综合管理、呼吸训练、综合促排痰干预联合四级康复训练、集束化护理模式、行为习惯逆转疗法结合照顾者参与健康教育模式以及预见性护理模式,从而促进患者病情恢复。To summarize the nursing case of an multimorbidity elderly patient with aspiration pneumonia,comprehensive geriatric assessment were conducted based on the patient's condition.For formulating a scientific and personalized nursing care plan,measures taken include comprehensive management of elderly patients in hospital with aspiration pneumonia,respiratory training,comprehensive promoting sputum removal intervention combined with four-level rehabilitation training,cluster nursing model,behavioural habit reversal therapy combined with caregiver participation in health education model,and anticipatory nursing care model to promote the recovery of the patient.
