摘　　要：目的系统评价石蜡疗法对肩手综合征患者的临床疗效。方法计算机检索中国知网(CNKI)、维普中文科技期刊数据库(VIP)、万方数据(Wanfang Data)及Web of Science、Embase、PubMed、Cochrane Library、Ovid MEDLINE等数据库中石蜡疗法对肩手综合征患者进行干预的临床随机对照试验论文,检索时间均为自建库至2024年5月12日。采用RevMan5.4统计软件进行数据的统计分析。结果共纳入9项研究共616例患者。Meta分析结果显示,石蜡疗法能有效改善肩手综合征患者运动功能(SMD=0.77,95%CI:0.60~0.95,P<0.00001),缓解疼痛(SMD=-1.89,95%CI:-2.18~-1.60,P<0.00001)、肿胀(SMD=-2.40,95%CI:-4.64~-0.16,P=0.04),提高患者日常生活活动能力(SMD=7.05,95%CI:5.51~8.59,P<0.00001)。结论石蜡疗法对肩手综合征患者肢体运动功能及疼痛、肿胀、日常生活活动能力均具有改善作用,但上述研究仍需要更多大样本、高质量随机对照试验加以验证。Objective To systematically evaluate the efficacy of paraffin therapy in patients with shoulder-hand syndrome.Methods Randomized controlled trials(RCTs)on paraffin therapy for shoulder-hand syndrome were retrieved from databases including CNKI,VIP,Wanfang Data,Web of Science,Embase,PubMed,Cochrane Library,and Ovid MEDLINE from their inception to May 12,2024.Data were analyzed using RevMan 5.4 software.Results Nine studies involving 616 patients were included.Meta-analysis showed that paraffin therapy significantly improved motor function(SMD=0.77,95%CI:0.60-0.95,P<0.00001),alleviated pain(SMD=-1.89,95%CI:-2.18 to-1.60,P<0.00001)and swelling(SMD=-2.40,95%CI:-4.64 to-0.16,P=0.04),and enhanced activities of daily living(SMD=7.05,95%CI:5.51-8.59,P<0.00001),with all differences being statistically significant(P<0.05).Conclusion Paraffin therapy demonstrates beneficial effects on motor function,pain,swelling,and activities of daily living in patients with shoulder-hand syndrome.However,further large-sample,high-quality RCTs are needed to validate these findings.
