经阴道超声检查联合经腹部超声检查在异位妊娠诊断中的效能

摘　　要：目的:分析经阴道超声检查(TVS)联合经腹部超声检查(TAS)在异位妊娠诊断中的效能。方法:选取2021年8月至2023年9月该院收治的96例疑似异位妊娠患者进行前瞻性研究,所有患者均行TVS、TAS检查。以病理检查结果为金标准,比较TVS、TAS单项及联合检查诊断异位妊娠的结果,比较TVS、TAS单项及联合检查在异位妊娠诊断中的效能,分析TVS、TAS检查与病理检查结果的一致性。结果:金标准结果显示,96例疑似异位妊娠患者中,阳性72例,阴性24例;TVS检查结果显示,阳性66例,阴性30例;TAS检查结果显示,阳性62例,阴性34例;TVS、TAS联合检查结果显示,阳性72例,阴性24例;TVS、TAS联合检查异位妊娠的灵敏度、准确度、阴性预测值均高于TVS、TAS单项检查,差异有统计学意义(P<0.05);Kappa检验结果显示,TVS、TAS单项检查诊断异位妊娠的结果与病理检查结果的一致性尚可(Kappa=0.641、0.512,P=0.000、0.000);TVS、TAS联合检查诊断异位妊娠的结果与病理检查结果的一致性良好(Kappa=0.889,P=0.000)。结论:TVS、TAS联合检查在异位妊娠诊断中的效能高于二者单项检查。Objective:To analyze efficiency of transvaginal ultrasonography(TVS)combined with transabdominal ultrasonography(TAS)in diagnosis of ectopic pregnancy.Methods:A prospective study was conducted on 96 patients with suspected ectopic pregnancy admitted to the hospital from August 2021 to September 2023.All patients underwent TVS and TAS examinations.Using the results of pathological examination as the gold standard,the results of single and combined detection of TVS and TAS in the diagnosis of ectopic pregnancy were compared,and the efficiency of single and combined detection of TVS and TAS in the diagnosis of ectopic pregnancy were also compared.Further,the consistency of TVS,TAS single and combined detection with pathological examination results were analyzed.Results:The gold standard showed that among the 96 patients with suspected ectopic pregnancy,72 were positive and 24 were negative;TVS test showed that 66 cases were positive and 30 cases were negative;TAS test showed that 62 cases were positive and 34 cases were negative;and the combined detection of TVS and TAS showed that 72 cases were positive and 24 cases were negative.The sensitivity,accuracy and negative predictive value of combined detection of TVS and TAS in the diagnosis of ectopic pregnancy were higher than those of single detection,and the differences were statistically significant(P<0.05).Kappa test results showed that the consistency of single detection of TVS and TAS in the diagnosis of ectopic pregnancy was acceptable with pathological examination(Kappa=0.641,0.512,P=0.000,0.000);while the combined detection of TVS and TAS had good consistency with pathological examination(Kappa=0.889,P=0.000).Conclusions:The efficiency of combined detection of TVS and TAS in the diagnosis of ectopic pregnancy is higher than that of single detection.
