摘 要:目的:分析血小板反应蛋白-1(TSP-1)、基质金属蛋白酶-9(MMP-9)、可溶性纤维蛋白单体复合物(SFMC)水平与子痫前期(PE)病情严重程度的相关性及对不良妊娠结局的预测价值。方法:回顾性分析2021年8月至2023年8月该院收治的70例PE患者的临床资料,设为观察组,另选取同期70名健康孕妇作为对照组。比较两组、不同病情严重程度及不同妊娠结局PE患者血清TSP-1、MMP-9、SFMC水平,采用Pearson相关性分析血清TSP-1、MMP-9、SFMC水平与PE患者病情严重程度的相关性,绘制受试者工作特征(ROC)曲线分析TSP-1、MMP-9、SFMC水平单项及联合检测预测不良妊娠结局的价值。结果:观察组TSP-1、SFMC水平均高于对照组,MMP-9水平低于对照组,差异有统计学意义(P<0.05);重度患者TSP-1、SFMC水平高于轻度患者,MMP-9水平低于轻度患者,差异均有统计学意义(P<0.05);经Pearson相关性分析结果显示,TSP-1、SFMC水平与PE患者病情严重程度均呈正相关(r>0,P<0.05),MMP-9水平与PE患者病情严重程度呈负相关(r<0,P<0.05);结局不良患者TSP-1、SFMC水平均高于结局良好患者,MMP-9水平低于结局良好患者,差异有统计学意义(P<0.05);ROC曲线分析结果显示,TSP-1、MMP-9、SFMC水平单项及联合检测预测不良妊娠结局的曲线下面积分别为0.837、0.751、0.789、0.946,且联合检测预测不良妊娠结局的价值高于三者单项检测。结论:TSP-1、SFMC水平与PE患者病情严重程度均呈正相关,MMP-9水平与PE患者病情严重程度呈负相关,TSP-1、MMP-9、SFMC水平联合检测预测不良妊娠结局的价值高于三者单项检测。Objective:To analyze correlation between thrombospondin-1(TSP-1),matrix metalloproteinase-9(MMP-9),soluble fibrin monomer complex(SFMC)levels and severity of preeclampsia(PE)and their predictive value for adverse pregnancy outcomes.Methods:A total of 70 patients with PE admitted to this hospital from August 2021 to August 2023 were retrospectively analyzed and they were set as the observation group,and 70 healthy pregnant women were selected as the control group.The levels of TSP-1,MMP-9 and SFMC were compared between the two groups,as well as PE patients with different disease severity and pregnancy outcomes.Pearson correlation analysis was used to analyze correlation between the TSP-1,MMP-9,SFMC levels and the disease severity in the PE patients.The receiver operating characteristic(ROC)curve was drawn to analyze the value of single and combined detection of TSP-1,MMP-9 and SFMC levels in the prediction of adverse pregnancy outcomes.Results:The levels of TSP-1 and SFMC in the observation group were higher than those in the control group,the level of MMP-9 was lower than that in the control group,and the differences were statistically significant(P<0.05).The levels of serum TSP-1 and SFMC in the severe patients were higher than those in the mild patients,the level of MMP-9 was lower than that in the mild patients,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the levels of TSP-1 and SFMC were positively correlated with the PE patients severity of the disease(r>0,P<0.05),and the level of MMP-9 was negatively correlated with the PE patients severity of the disease(r<0,P<0.05).The levels of serum TSP-1 and SFMC in the patients with poor outcomes were higher than those in the patients with good outcomes,the level of serum MMP-9 was lower than that in the patients with good outcomes,and the differences were statistically significant(P<0.05).The ROC curve analysis showed that the area under the curve of single and combined detection of TSP-1,MMP-9 and SFMC levels in
