基于跨理论模型的协同护理在高血压心脏病患者中的应用效果

摘　　要：目的:观察基于跨理论模型的协同护理在高血压心脏病患者中的应用效果。方法:回顾性分析2021年8月至2022年9月新乡市中心医院收治的96例高血压心脏病患者的临床资料,根据护理方法不同将其分为对照组和观察组各48例。对照组给予常规护理,观察组在对照组基础上给予基于跨理论模型的协同护理,比较两组锻炼依从率,护理前后6分钟步行试验(6MWT)距离、自我效能感[一般自我效能感量表(GSES)]评分、生命质量[中国心血管病人生活质量评定问卷(CQQC)]评分,以及护理满意度。结果:观察组锻炼依从率为100.00%(48/48),高于对照组的87.50%(42/48),差异有统计学意义(P<0.05);护理后,两组6MWT距离均长于护理前,且观察组长于对照组,差异有统计学意义(P<0.05);护理后,两组GSES、CQQC评分均高于护理前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组护理满意度为97.92%(47/48),高于对照组的83.33%(40/48),差异有统计学意义(P<0.05)。结论:在常规护理基础上给予基于跨理论模型的协同护理可提高高血压心脏病患者锻炼依从率、GSES评分、CQQC评分和护理满意度,延长6MWT距离,效果优于单纯常规护理。Objective:To observe application effects of collaborative nursing based on trans-theoretical model in patients with hypertensive heart disease.Methods:The clinical data of 96 patients with hypertensive heart disease admitted to Xinxiang central hospital from August 2021 to September 2022 were retrospectively analyzed.According to different nursing methods,they were divided into control group and observation group,48 cases in each group.The control group was given routine nursing,while the observation group was given the collaborative nursing based on transtheoretical model on the basis of the control group.The exercise compliance rate,the 6-minute walking test(6MWT)distance before and after the nursing,the self-efficacy[general self-efficacy scale(GSES)]score,the quality of life[Chinese cardiovascular patient quality of life questionnaire(CQQC)]score,and the nursing satisfaction were compared between the two groups.Results:The exercise compliance rate of the observation group was 100.00%(48/48),which was higher than 87.50%(42/48)of the control group,and the difference was statistically significant(P<0.05).After the nursing,the 6MWT distance of the two groups were longer thanthose before the nursing,that of the observation group was longer than that of the control group,and the differences were statistically significant(P<0.05).After the nursing,the scores of GSES and CQQC in the two groups were higher than those before the nursing,those in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,the nursing satisfaction of the observation group was 97.92%(47/48),which was higher than 83.33%(40/48)of the control group,and the difference was statistically significant(P<0.05).Conclusions:The collaborative nursing based on trans-theoretical model on the basis of the routine nursing can improve the exercise compliance rate,the GSES scores,the CQQC scores and the nursing satisfaction,extend the 6MWT distance of the patients with hypertensive he
