腹腔镜袖状胃切除术与胃旁路术治疗肥胖合并糖尿病的效果比较

摘　　要：目的比较腹腔镜袖状胃切除术(LSG)与胃旁路术(LRYGB)治疗肥胖合并糖尿病的效果。方法回顾性收集本院2019年3月至2024年4月接收的54例肥胖合并2型糖尿病患者病历资料,依据手术方法分为LSG组(29例)与LRYGB组(25例)。比较两组手术指标,术后6个月减重情况,糖脂代谢指标。结果LSG组手术用时少于LRYGB组(P<0.05),两组出血量、排气时间、进食时间、住院时间、并发症比较差异无统计学意义(P>0.05)。两组术后腰围、体重指数及体重均少于术前(P<0.05),但两组间比较差异无统计学意义(P>0.05)。两组术后糖脂代谢指标均低于术前(P<0.05),但两组间比较差异无统计学意义(P>0.05)。结论肥胖合并糖尿病患者采用LSG及LRYGB治疗均有良好的短期效果,均可有效改善患者体重、糖脂代谢情况,但LSG的手术用时更短。Objective To compare the efficacy of laparoscopic sleeve gastrectomy(LSG)and Laparoscopic Roux-en-Y Gastric Bypass(LRYGB)in the treatment of obesity with diabetes.Methods The medical records of 54 obese patients with type 2 diabetes admitted to our hospital from March 2019 to April 2024 were retrospectively collected,according to the surgical method,they were divided into LSG group(29 cases)and LRYGB group(25 cases).Two sets of surgical indicators,including weight loss and glucose and lipid metabolism 6 months after surgery were compared.Results The surgery time in the LSG group was less than that in the LRYGB group(P<0.05),and there were no significant difference in bleeding volume,exhaust time,feeding time,hospitalization time,and complications between the two groups(P>0.05).The waist circumference,body mass index,and body weight of both groups were lower than before surgery(P<0.05),but there was no significant difference between the two groups(P>0.05).Both postoperative glucose and lipid metabolism indicators were lower than preoperative levels(P<0.05),but there was no significant difference between the two groups(P>0.05).Conclusion Both LSG and LRYGB have good short-term effects on obese patients with diabetes,which can effectively improve their weight,glucose and lipid metabolism,but the operation time of LSG is shorter.
