摘 要:目的基于功能磁共振成像(fMRI)低频振幅(ALFF)探讨八段锦对慢性失眠(CI)疲劳患者脑部神经元自发活动的调节作用。方法选取30例自愿参与fMRI检测的CI患者随机分配到功法组和对照组,分别进行八段锦练习和认知行为疗法,疗程均为8周。在干预前后进行查尔德疲劳量表(FS-14)评价,扫描头部静息态功能磁共振成像(rs-fMRI),检测ALFF神经元活动强弱,并对八段锦干预前后FS-14量表差值与脑ALFF差值进行相关性分析。结果与治疗前相比,两组受试者治疗后FS-14评分均显著降低,差异具有统计学意义(P<0.05)。与治疗前相比,功法组ALFF值升高的区域是眶部额下回、眶部额上回、壳核、额上回;与对照组相比,功法组ALFF值显著升高的脑区是左侧枕下回、右侧额上回、左侧顶上回,显著降低的脑区是左侧颞中回、右侧额中回、左侧中央旁小叶。功法组干预前后FS-14差值与眶部额下回、眶部额上回、额上回ALFF差值显著负相关(P<0.05);功法组干预前后FS-14躯体疲劳差值与内部额上回ALFF差值显著负相关(P<0.05)。结论八段锦能改善CI患者的疲劳程度,且存在脑功能区神经元活动异常,fMRI能对CI疗效评估和判断提供一定的帮助。Objective To explore the regulatory effects of Baduanjin exercise on the spontaneous neuronal activity in the brains of patients with chronic insomnia(CI)-related fatigue based on the amplitude of low-frequency fluctuation(ALFF)of functional magnetic resonance imaging(fMRI).Methods Thirty CI patients who voluntarily participated in fMRI detection were randomly assigned to the Baduanjin group and the control group,and received Baduanjin exercise and cognitive behavioral therapy,respectively,for 8 weeks.The Fatigue Scale-14(FS-14)was used to evaluate fatigue before and after the intervention,and resting-state fMRI(rs-fMRI)was performed to detect the strength of neuronal activity(ALFF).The correlation between the changes in FS-14 scores and brain ALFF values before and after Baduanjin intervention was analyzed.Results Compared with before treatment,the FS-14 scores of both groups were significantly reduced after treatment(P<0.05).Compared with before treatment,the Baduanjin group showed increased ALFF values in the inferior frontal gyrus,superior frontal gyrus,nucleus accumbens,and superior medial frontal gyrus.Compared with the control group,the Baduanjin group showed significantly increased ALFF values in the left inferior occipital gyrus,right superior frontal gyrus,and left superior parietal lobule,and significantly decreased ALFF values in the left middle temporal gyrus,right middle frontal gyrus,and left paracentral lobule.The changes in FS-14 scores and ALFF values in the inferior frontal gyrus,superior frontal gyrus,and superior medial frontal gyrus were significantly negatively correlated(P<0.05)in the Baduanjin group.The changes in FS-14 somatic fatigue scores and ALFF values in the superior medial frontal gyrus were significantly negatively correlated(P<0.05)in the Baduanjin group.Conclusion Baduanjin can improve the fatigue level of CI patients,and there are abnormalities in neuronal activity in certain brain functional areas.fMRI can provide certain help for the evaluation and judgment of the therapeuti
