运脾散穴位贴敷对脾胃虚弱型糖尿病胃轻瘫患者主要症状指数及血糖水平的影响

摘　　要：目的探究运脾散穴位贴敷对脾胃虚弱型糖尿病胃轻瘫患者胃轻瘫主要症状指数(GCSI)的干预效果,同时监测干预后患者血糖水平的波动情况。方法选取2023年7月至2024年7月福建中医药大学附属福州中医院内分泌科符合西医诊断标准2型糖尿病消化系统自主神经病变和中医诊断为“痞满”且辨证为脾胃虚弱型的52例糖尿病胃轻瘫住院患者,采用随机数字表法分组,观察组和对照组各26例。两组患者均进行常规护理,观察组在常规护理基础上采用运脾散穴位贴敷治疗,对照组采用安慰剂穴位贴敷安慰性治疗。评价两组患者干预前、干预2周后的GCSI、晨起空腹血糖、中医症候积分。结果经过2周的干预,两组患者的GCSI评分、空腹血糖水平和中医证候积分均呈现出下降的趋势,其中观察组GCSI评分下降至(15.62±6.96),低于对照组的(24.19±9.99)(P<0.05);观察组空腹血糖值在治疗2周后为(6.53±1.32),低于照组的(8.13±2.41)(P<0.05);观察组中医证候积分为(11.89±7.21),远低于对照组的(20.15±5.23)(P<0.05)。观察组治疗总有效率为88.46%(23/26),高于对照组的34.61%(9/26)(P<0.05)。结论运脾散穴位贴敷治疗脾胃虚弱型糖尿病胃轻瘫可有效缓解脾患者胃部不适症状,促进胃肠蠕动、排空,同时还具调节患者血糖水平的作用。Objective To explore the intervention effect of Yunpi Powder acupoint application on the gastroparesis cardinal symptom index(GCSI)in diabetic gastroparesis patients with spleen and stomach weakness,and to monitor thefluctuation of blood sugar levels in patients after the intervention.Methods A total of 52 inpatients with diabetic gastroparesis who met the Western medical diagnostic criteria for type 2 diabetes mellitus with gastrointestinal autonomic neuropathy and were diagnosed with"Piman"in traditional Chinese medicine(TCM)with spleen-stach deficiency syndrome were selected from the Department of Endocrinology,Fuzhou Traditional Chinese Medicine Hospital Affiliated to Fujian University of Traditional Chinese Medicine from 2023 to July 2024.The patients were divided into an observation group(26 cases)and a control group(26 cases)according to the random number table method.Both groups of patients received routine care.The observation group was treated with Yunpi Powder acupoint application on the basis of routine care,while the control group was treated with placebo acupoint application as comfort treatment.The GCSI,morning fasting blood glucose levels,and TCM symptom scores of the two groups of patients before the intervention and 2 weeks after the intervention were evaluated.Results After two weeks of intervention,both groups of patients showed a downward trend in GCSI scores,fasting blood glucose levels,and TCM syndrome scores.Specifically,the GCSI score in the observation group decreased to(15.62±6.96),which was lower than that in the control group(24.19±9.99)(P<0.05).After two weeks of treatment,the fasting blood glucose level in the observation group was(6.53±1.32),which was lower than that in the control group(8.13±2.41)(P<0.05).After two weeks of treatment,the TCM syndrome score in the observation group was(11.89±7.21),significantly lower than that in the control group(20.15±5.23)(P<0.05).The overall effective rate of treatment in the observation group was 88.46%(23/26),which was higher than the c
