摘　　要：目的通过Meta分析比较单通道空肠间置术和双通道空肠间置术在近端胃切除术后消化道重建效果。方法分别以中文、英文检索词检索国内外医学数据库,收集对比单通道空肠间置术和双通道空肠间置术的文献,使用Rev Man 5.4对纳入入选研究的围手术期指标、术后并发症率、术后6个月营养指标进行Meta分析。结果共11篇文献纳入研究,围手术期方面(手术过程用时、术中失血量、手术后肠道恢复排气时间及术后住院时长)和术后并发症的发生率上,两种术式之间的比较并未得出统计学差异。术后6个月营养状况上,单通道空肠间置术相较于双通道空肠间置术显示出一定的优势。结论在长期预后方面,本Meta分析发现单通道空肠间置术相较于双通道空肠间置术显示出一定的优势。这主要体现在患者的营养状况上(白蛋白、血红蛋白、体重)。Objective To compare the effectiveness of single tract jejunal reconstruction and double tract jejunal reconstruction after proximal gastrectomy.Methods Searching for clinical trials that comparing single tract jejunal reconstruction and double tract jejunal reconstruction on PubMed Embase,Cochrane library,China National Knowledge Infrastructure(CNKI),VIP Information Chinese Journal Service Platform,and Wanfang Data Knowledge Service Platform.Perioperative indicators,postoperative complication rates,and nutritional indicators at 6 months after surgery included are meta analyze by Revman 5.4.Results The 11 articles are included in the study,and there are no statistically significant difference between the two surgical procedures in terms of perioperative time(surgery time,intraoperative blood loss,postoperative hospital stay,postoperative exhaust time)and postoperative complications rates.In terms of nutritional indicators at 6 months after surgery,single tract jejunal reconstruction showed certain advantages compared to double tract jejunal reconstruction.Conclusions In terms of long-term prognosis,this meta-analysis reveals that single tract jejunal reconstruction demonstrates certain advantages compared to double tract jejunal reconstruction.These advantages are primarily reflected in the patients’nutritional status,including albumin levels,hemoglobin levels,and body weight.
