摘　　要：目的分析危机管理理论全局式护理对ICU重症心力衰竭患者预后的影响。方法选取2023年1月—12月厦门市第五医院的80例ICU重症心力衰竭患者,随机数字表法分为观察组(40例)和对照组(40例),对照组心力衰竭患者采用常规护理干预,观察组心力衰竭患者采用危机管理理论全局式护理,对比两组心力衰竭患者护理前后运动耐力(6 min步行距离,6MWT)、焦虑抑郁心理、心功能改善[左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)]。结果观察组患者LVEF及6MWT水平高于对照组,LVEDD、LVESD低于对照组(P<0.05)。观察组患者SAS、SDS心理状态评分低于对照组(P<0.05)。结论危机管理理论全局式护理对ICU重症心力衰竭患者的预后提升具有非常积极的影响,能更有效地改善患者负性情绪,提升患者的心功能,增强患者的运动耐力。Objective To analyze the impact of crisis management theory on the prognosis of ICU patients with severe heart failure.Methods A total of 80 patients with severe heart failure in ICU in Xiamen Fifth hospital from January 2023 to December 2023 were selected and randomly divided into observation group(40 cases)and control group(40 cases).Patients with heart failure in the control group received conventional nursing intervention,while patients with heart failure in the observation group received global nursing based on crisis management theory.Exercise endurance(6 min walking distance,6MWT),anxiety and depression,improvement of heart function(left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD)before and after care were compared between the two groups.Results LVEF and 6MWT levels in observation group were higher than control group,LVEDD and LVESD were lower than control group(P<0.05).The scores of SAS and SDS psychological states of the patients in the observation group were lower than those in the control group(P<0.05).Conclusions Global nursing based on crisis management theory has a very positive impact on improving the prognosis of ICU patients with severe heart failure.It can more effectively improve patients’negative emotions,enhance patients’cardiac function and strengthen patients’exercise tolerance.
