心形美容缝合与皮肤改良缝合对产后腹壁切口瘢痕及美观度的影响比较

摘　　要：目的探究心形美容缝合及皮肤改良缝合对产后腹壁切口瘢痕及美观度的影响。方法选取2021年9月至2023年9月收治的剖宫产产妇为研究对象,根据随机数字表法分为A组和B组,每组75例。A组进行心形美容缝合,B组进行皮肤改良缝合。记录产妇手术相关指标;比较术后切口疼痛情况、愈合情况差异;比较术后6个月时瘢痕情况(产妇与观察者瘢痕评价量表、瘢痕增生率、瘢痕宽度、瘢痕最薄厚度),以及切口美观度差异和并发症发生情况。结果A组缝合时间和切口愈合时间显著低于B组(P<0.05);术后7 d、30 d时,A组视觉模拟评分(VAS)评分均低于B组,A组切口愈合等级情况优于B组(P<0.05);术后6个月时,A组产妇与观察者瘢痕评价量表(POSAS)评分、瘢痕增生率、瘢痕宽度、瘢痕最薄厚度均显著低于B组(P<0.05),A组美观度显著高于B组(P<0.05);A组术后并发症发生率显著低于B组(P<0.05)。结论剖宫产切口进行心形美容缝合有助于术后切口愈合,可有效减少切口瘢痕生成,且具有较高的美容效果。Objective To explore the influence of heart-shaped cosmetic suture and skin modified suture on scar and aesthetic degree of postpartum abdominal wall incision.Methods Puerperae with cesarean section admitted from September 2021 to September 2023 were selected as the research subjects.According to the random number table method,the patients were classified into group A and group B,with 75 cases in each group.Group A received heart-shaped cosmetic suture,while group B was given skin modified suture.The surgery-related indicators of puerperae were recorded.The differences in postoperative incision pain[visual analogue score(VAS)]and healing were compared between the two groups of patients.The differences in scar status[patient and observer scar assess-mentscale(POSAS),scar hyperplasia rate,scar width,the thinnest scar thickness]and incision aesthetic degree at 6 months after surgery were compared,and the complications were recorded.Results The suture time and incision healing time in group A were significantly shorter than those in group B(P<0.05).At 7 and 30 days after surgery,the VAS score in group A was lower than that in group B,and the incision healing grading was better than that in group B(P<0.05).The POSAS scores,scar hyperplasia rate,scar width and the thinnest scar thickness in group A at 6 months after surgery were significantly lower than those in group B(P<0.05),and the effective rate of aesthetic degree was significantly higher than that in group B(P<0.05).The incidence rates of postoperative complications in group A were significantly lower compared to group B(P<0.05).Conclusion Heart-shaped cosmetic suture during cesarean section is helpful for postoperative incision healing,and can effectively reduce incision scar formation with a high cosmetic effect.
