摘 要:目的分析2017—2022年重庆市基层医疗卫生资源配置的发展趋势及公平性,为制定卫生政策、优化资源配置提供参考。方法收集2017—2022年重庆市基层医疗卫生机构资源数据,分析资源配置情况及变化趋势,并利用基尼系数和泰尔指数进行公平性分析。结果2017—2022年重庆市基层医疗卫生机构数、床位数、卫生人员、执业(助理)医师、注册护士分别增加了2478个、8197张、23885人、13987人和14601人,年均增长速度分别为2.52%、3.03%、5.10%、8.72%和13.06%。按人口分布的基尼系数在0.0718~0.3356之间,泰尔指数在0.0036~0.1024之间,公平性较好;按面积分布的基尼系数在0.3138~0.5913之间,泰尔指数范围为0.0719~0.2717,大部分资源公平性较差,不公平性来自区域间。结论重庆市基层医疗卫生资源整体呈上升趋势,但仍存在缺乏,且资源配置的不公平性也在逐年增加。应进一步优化基层医疗卫生机构和人员的配备,推动区域间基层医疗卫生资源的协同发展。Objective To analyzes the trend and equity of health resource allocation in primary healthcare institutions in Chongqing from 2017 to 2022,so as to provide reference for formulating relevant health policies and optimizing the resource allocation.Methods We collected the number of primary health and health resources in Chongqing from 2017 to 2022.Descriptive analysis was used to evaluate the resource allocation and the trends,and Gini coefficient and Theil index were used to evaluate the equity of allocation.Results From 2017 to 2022,the number of primary healthcare institutions,beds,health workers,practicing(assistant)physicians,and registered nurses increased by 2478,8197,23885,13987,and 14601 respectively,with an annual growth rate of 2.52%,3.03%,5.10%,8.72%,and 13.06%respectively.According to the population distribution,the Gini coefficient was between 0.0718 and 0.3356,and the Theil index was between 0.0036 and 0.1024,which represented basically fair.Meanwhile,the Gini coefficient varied between 0.3138 and 0.5913 for geographic distribution,while the Theil index was between 0.0719 and 0.2717,which indicated unfairness in most of the resources.The regional difference was the major reason for the unbalance in health resource allocation.Conclusions The number of primary health resources in Chongqing is on the rise,but there is still a lack of them,and the inequality of resource allocation is increasing year by year.It is necessary to further optimize the primary healthcare institutions and health human resource,in order to promote the collaborated development of primary health resources among the regions.
