摘　　要：目的全面了解年轻妇科癌症患者社会疏离的真实体验。方法采用目的抽样法,选取2023年6—8月在武汉市某三甲医院妇科肿瘤病房治疗的患者作为研究对象,对符合纳排标准的19例年轻妇科癌症患者进行半结构式访谈。采用Colaizzi内容分析法对资料进行分析与主题提炼。结果通过对访谈资料的分析归纳,提炼汇总为3个主题:负性情绪体验(孤独感与无助感、病耻感与自卑心理、无意义感与缺乏社交兴趣)、客观行为改变(以保护自我为目的的自我承受、以保护他人为目的的隐匿心声、沟通交流障碍)、外界支持不足(医护专业支持不足、家庭及亲友支持不良、同伴支持受限)。结论年轻妇科癌症患者正面临较高的社会疏离。医护人员应结合患者的实际情况及个体化因素,制定针对性干预方案;加强患者的心理支持、医疗信息支持及健康指导,调动全方位社会支持系统,促进患者积极融入社会,早日回归正常生活。Objective To comprehensively understand the real experiences of social isolation in young patients with gynecological malignant tumors.Methods From June to August 2023,purpose sampling method was used to select patients treated in the gynecological oncology ward of a Grade A hospital in Wuhan as the research object,and 19 young patients with gynecological cancer who met the inclusion and exclusion criteria were conducted a semi-structured interviews.Colaizzi's content analysis method was employed to analyze and refine themes from the data.Results Through analysis and summarization of interview data,3 themes were identified:Negative emotional experiences(feelings of loneliness and helplessness,disease-related shame and low self-esteem,and sense of meaninglessness and lack of social interest),objective behavioral changes(self-protection with a focus on oneself,hidden voices aimed at protecting others,and communication barriers),and insufficient external support(lack of professional medical support,inadequate family and friends'support,and limited peer support).Conclusion Young gynecologic cancer patients are facing higher levels of social isolation.Healthcare professionals should develop targeted intervention plans based on individualized factors considering each patient's specific circumstances,strengthen psychological support,medical information support,and health guidance for patients,and mobilize comprehensive social support systems to promote patients to actively integrate into society and return to normal life as soon as possible.
