游离大鱼际皮瓣修复手指软组织缺损的临床疗效

摘　　要：目的探讨游离大鱼际穿支皮瓣修复手指软组织缺损的临床疗效。方法收集2019年3月至2021年3月在梧州市人民医院创伤骨科,采用游离大鱼际皮瓣移植修复手部软组织缺损的患者32例,其中男23例,女9例,年龄19~56岁。创面皮肤软组织缺损面积在1.5 cm×2.4 cm~2.1 cm×3.6 cm之间,26例一期急诊行游离大鱼际皮瓣修复创面;6例急诊先行手指创面清创封闭负压引流,二期行游离大鱼际皮瓣修复创面。术后观察皮瓣存活情况,随访1~2年,评估皮瓣外观、质地、感觉情况。结果术后皮瓣全部成活,术后经过1~2年随访,皮瓣质地薄,弹性、感觉恢复良好,两点辨别觉为3~8 mm。结论采用游离大鱼际皮瓣修复手指软组织缺损临床效果确切,是修复手指皮肤缺损的有效方法,临床中值得推广应用。Objective To investigate the clinical efficacy of free thenar perforator flap for the repair of soft tissue defects in fingers.Methods From March 2019 to March 2021,32 cases of soft tissue defects of hand were repaired by free large thenar flap transplantation in the Department of Traumatic Orthopedics at Wuzhou People's Hospital.The study included 23 males and 9 females,aged 19-56 years.The skin and soft tissue defect area ranged from 1.5 cm×2.4 cm to 2.1 cm×3.6 cm.Among them,26 patients were repaired by free thenar flap in the first visit of emergency department,while 6 patients received free thenar flap in the second visit of emergency department.Survival of the skin flap was observed after surgery,and the appearance,texture and feeling of the skin flap were evaluated during the 1-2 years of follow-up.Results All flaps survived postoperatively.During the 1-2 years of follow-up period,the flaps demonstrated thin texture,good elasticity,and satisfactory sensory recovery,with twopoint discrimination of 3-8 mm.Conclusion Free large thenar flap repair for finger soft tissue defects is clinically effective,offering a reliable method for repairing finger skin defects.It is worth promoting in clinical practice.
