小针刀联合关节腔注射治疗冻结期肩周炎的临床疗效

摘　　要：目的:评估小针刀与关节腔注射联合治疗方案对冻结期肩周炎患者的临床疗效。方法:通过计算机随机数字表法将来自安徽省针灸医院骨一科的72例冻结期肩周炎患者分为对照组和研究组,每组36例。对照组患者肩关节腔注入2%盐酸利多卡因注射液2 ml+复方倍他米松注射液1 ml+0.9%氯化钠注射液5 ml的混合液体,治疗1次/周,共治疗3次。研究组患者在对照组治疗基础上联合小针刀治疗,在喙突点、肱骨大小结节、肱骨结节间沟、肩峰下滑囊以及2~3个压痛最明显的点行纵行疏通法和横行剥离法2~3次,1次/周,共治疗3次。比较两组患者治疗后的临床疗效、疼痛视觉模拟量表(VAS)评分、Constant-Murley肩关节功能评分。结果:研究组治疗总有效率高于对照组,差异有统计学意义(P<0.01)。治疗后两组患者VAS评分均下降,Constant-Murley肩关节功能评分均提高,且研究组VAS评分、Constant-Murley肩关节功能评分均优于对照组,差异均有统计学意义(P<0.01)。结论:将小针刀疗法与关节腔注射相结合,在减轻冻结期肩周炎患者疼痛和增强关节活动范围方面取得了良好的临床疗效。Objective:To assess the clinical efficacy of small needle-knife combined with intra-articular injection in the treatment of periarthritis of frozen shoulder.Methods:72 patients with periarthritis of frozen shoulder from the First Department of Orthopedics of Anhui Acupuncture and Moxibustion Hospital were divided into control group and study group by computer random number method,with 36 cases in each group.The patients in the control group were injected with mixed liquid of 2%lidocaine hydrochloride injection 2 ml+compound betamethasone injection 1 ml+0.9%sodium chloride injection 5 ml into the shoulder joint cavity,once a week,for a total of 3 times.On the basis of the treatment of the control group,the patients in the study group were treated with small needle-knife.The longitudinal dredge method and transverse dissection method were performed at the coracoid point,large and small tubercles of humerus,intertubercular groove of humerus,subacromial bursa,and 2-3 points with the most obvious tenderness for 2 to 3 times,once a week,a total of 3 times.The clinical efficacy,pain visual analogue scale(VAS)score and Constant-Murley shoulder function score were compared between the two groups after treatment.Results:The total effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.01).After treatment,the VAS scores of the two groups decreased,and the Constant-Murley shoulder joint function score increased.The VAS score and Constant-Murley shoulder joint function score of the study group were better than those of the control group,and the differences were statistically significant(P<0.01).Conclusion:The combination of small needle-knife and intra-articular injection has achieved good clinical efficacy in relieving pain and enhancing joint range of motion in patients with periarthritis of frozen shoulder.
