摘　　要：目的:根据同通用名或同类产品的口服与静脉注射2种剂型用于治疗同一疾病的药物经济学评价文献,分析和讨论如何量化药物经济学评价研究中剂型不同带来的差异。方法:计算机检索中国知网、万方数据、维普网、PubMed、Wiley online library等数据库,收集口服与静脉注射剂型的药物经济学评价相关文献,并对符合纳排标准的文献进行整理与总结。结果:共有18篇文献纳入研究,研究提示了相比静脉注射剂型,口服剂型不仅可以节省静脉注射器、输液管等耗材的费用,减少护理时间节省人力资源,还可在居家治疗的情景下节省就医交通费和误工损失。此外,部分研究还考虑了口服剂型提高患者健康效用值与用药依从性,并将其健康收益纳入经济学评价的计算中。结论:剂型差异会带来一定的成本、疗效和不良反应发生率方面的差异,这些差异应该在药物经济学评价中合理量化并体现出来。除此之外,治疗情景不同(如住院静脉注射与居家口服用药)的情况下,不同剂型、给药方式患者的治疗体验差异较大,在药品评价研究中应纳入计算。Objective:Based on the literatures on pharmacoeconomic evaluation of oral and intravenous dosage forms of drugs with the same chemical name or category for the treatment of the same disease,this article analyzes and discusses how to quantify the differences caused by different dosage forms in pharmacoeconomic evaluation.Methods:The relevant literature on pharmacoeconomic evaluation of oral and intravenous dosage forms was collected from online database CNKI,Wanfang Data,Vip,PubMed,Wiley online library,etc.,and the literature meeting the standard of inclusion and exclusion criteria was sorted out and summarized.Results:A total of 18 literatures were included in the study.The result suggested that compared with the intravenous dosage form,the oral dosage form not only saves the cost of consumables such as intravenous syringes and infusion tubes,reduce nursing time and save human resources,but also saves the cost of medical transportation and loss of work hour under the scenario of home treatment.In addition,some studies also considered that oral dosage forms can improve the patients health effectiveness and medication compliance,thus their health benefits were included in the calculation of economic evaluation.Conclusion:Differences in dosage forms may lead to differences in cost,efficacy and incidence of complications,which should be reasonably quantified and reflected in pharmacoeconomic evaluation.In addition,in the case of different treatment scenarios(such as inpatient intravenous injection and home oral administration),the treatment experience of patients with different dosage forms may be remarkably different,which should be included in the calculation of drug evaluation studies.
