当归六黄汤合苍附导痰丸加减辅治多囊卵巢综合征不孕痰湿内阻型的临床价值

摘　　要：目的 分析多囊卵巢综合征(polycystic ovary syndrome,PCOS)不孕痰湿内阻型患者实施当归六黄汤合苍附导痰丸加减治疗的效果。方法 选取2022年11月—2023年11月临沂市中医医院收治的84例PCOS不孕患者为研究对象,均为痰湿内阻型,按照不同治疗的方法分为对照组、研究组,各42例。对照组行常规治疗,研究组在常规治疗基础上辅以当归六黄汤合苍附导痰丸加减治疗。对比两组激素指标、恢复指标、临床疗效和远期预后。结果 治疗后,研究组卵泡刺激素、睾酮、黄体生成素、黄体生成素/卵泡刺激素水平均低于对照组,差异有统计学意义(P均<0.05)。治疗后,研究组症状积分低于对照组,子宫内膜厚度高于对照组,差异有统计学意义(P均<0.05)。研究组治疗总有效率为97.62%(41/42),高于对照组的83.33%(35/42),差异有统计学意义(χ^(2)=4.974,P=0.026)。随访1年后,研究组正常排卵率、妊娠率均高于对照组,差异有统计学意义(P均<0.05)。结论 当归六黄汤与苍附导痰丸加减治疗PCOS不孕的预后较好,能纠正激素代谢状态,改善子宫内膜厚度,改善临床症状。Objective To analyze the effect of Danggui Liuhuang Decoction and Cangfu Daotan Pill in the treatment of polycystic ovary syndrome(PCOS) infertility patients with phlegm-dampness internal obstruction.Methods A total of 84 patients with PCOS infertility admitted to Linyi Hospital of Traditional Chinese Medicine from November 2022 to November 2023 were selected as the research objects.All of them were of phlegmdampness internal obstruction type.According to different treatment methods,they were divided into control group and study group,with 42 cases in each group.The control group was treated with routine treatment,and the study group was treated with Danggui Liuhuang Decoction and Cangfu Daotan Pill on the basis of routine treatment.The hormones indexs,recovery indexes,clinical efficacy and long-term prognosis of the two groups were compared.Results After treatment,the levels of follicle stimulating hormone,testosterone,luteinizing hormone and luteinizing hormone/follicle stimulating hormone in the study group were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the symptom score of the study group was lower than that of the control group,and the endometrial thickness was higher than that of the control group,the differences were statistically significant(both P<0.05).The total effective rate of treatment in the study group was 97.62%(41/42),which was higher than 83.33%(35/42) in the control group,and the difference was statistically significant(χ~2=4.974,P=0.026).After 1 year of follow-up,the normal ovulation rate and pregnancy rate of the study group were higher than those of the control group,and the differences were statistically significant(both P<0.05).Conclusion Danggui Liuhuang Decoction and Cangfu Daotan Pill have a good prognosis in the treatment of PCOS infertility,which can correct hormone metabolism,improve endometrial thickness and improve clinical symptoms.
