膝关节后交叉韧带止点撕脱骨折患者采用关节镜下手术治疗的效果分析

摘　　要：目的 探讨膝关节后交叉韧带止点撕脱骨折患者实施关节镜下手术治疗的临床疗效。方法选取2019年4月—2022年4月广西钦州市中医医院治疗的72例膝关节后交叉韧带止点撕脱骨折患者,基于不同的治疗方法进行分组,每组36例。对照组采用切开复位固定术治疗,观察组采用关节镜下手术治疗。比较两组的切口愈合率、术后6个月骨折愈合率、膝关节Lysholm评分(Lysholm Knee Score,LKS)、主观膝部评估表评分、并发症发生率。结果 观察组切口愈合率为61.11%(22/36),高于对照组的41.67%(15/36),差异有统计学意义(χ^(2)=5.094,P均<0.05)。观察组术后6个月骨折愈合率高于对照组,观察组治疗后的LKS评分、主观膝部评估表评分均高于对照组,观察组并发症发生率低于对照组,差异有统计学意义(P均<0.05)。结论 交叉韧带止点撕脱骨折患者实施关节镜下手术治疗可以缓解疼痛,稳定体征,提高切口、骨折愈合率,利于患者膝关节功能增强,且安全性好。Objective To investigate the clinical efficacy of arthroscopic surgery in patients with avulsion fracture of posterior cruciate ligament of knee joint.Methods From April 2019 to April 2022,72 patients with avulsion fracture of posterior cruciate ligament of knee joint admitted to Qinzhou Hospital of Traditional Chinese Medicine were selected and divided into groups based on different treatment methods,with 36 cases in each group.The control group was treated with open reduction and fixation,and the observation group was treated with arthroscopic surgery.The incision healing rate,fracture healing rate at 6 months after operation,Lysholm Knee Score(LKS),the International Knee Documentation Committee Subjective Knee Form and incidence of complications were compared between the two groups.Results The wound healing rate of the observation group was 61.11%(22/36),which was higher than 41.67%(15/36) of the control group,and the difference was statistically significant(χ~2=5.094,P<0.05).The fracture healing rate 6 months after operation in the observation group was higher than that in the control group,the LKS score and the International Knee Documentation Committee Subjective Knee Form score after treatment in the observation group were higher than those of the control group,and the incidence of complications in the observation group was less than that in the control group,the differences were statistically significant(all P<0.05).Conclusion For patients with avulsion fracture of cruciate ligament,arthroscopic surgery can relieve pain,stabilize signs,increase incision and fracture healing rate,enhance knee joint function and has good safety.
