四维彩色多普勒超声联合唐氏筛查指标诊断胎儿生长受限的临床价值

摘　　要：目的 探讨四维彩色多普勒超声联合唐氏筛查应用于胎儿生长受限的诊断价值。方法 选取2020年5月—2021年5月在菏泽市妇幼保健院就诊的50例胎儿生长受限孕妇,以及同期50例进行常规孕检的正常妊娠孕妇为研究对象,分别作为研究组、对照组。对比两组血管化指数(vascularization index,VI)、血流指数(flow index,FI)、血管化血流指数(vascularization flow index,VFI),以及游离雌三醇(unconjugated estriol,uE3)、甲胎蛋白(alpha-fetoprotein,AFP)及游离β-绒毛膜促性腺激素(beta-human chorionic gonadotropin,β-hCG)生物标志物的含量,以受试者操作特征(receiver operating characteristic,ROC)曲线评价四维彩色多普勒超声联合唐氏筛查对胎儿生长受限的诊断效能。结果对照组多普勒超声参数VI、FI、VFI分别为14.39±2.25、20.38±2.15、4.08±0.63,均低于研究组的28.55±4.17、32.62±1.77、8.25±1.40,差异有统计学意义(t=21.131、31.079、20.450,P均<0.05)。研究组uE3、AFP、β-hCG均高于对照组,差异有统计学意义(P均<0.05)。ROC曲线分析显示,四维彩色多普勒超声联合唐氏筛查检测的灵敏度和特异度更优。结论 四维彩色多普勒超声联合唐氏筛查为胎儿生长受限的早期检测与评估提供了更为准确和全面的临床信息,诊断效能较高。Objective To explore the value of four-dimensional color Doppler ultrasound combined with Down's screening in the diagnosis of fetal growth restriction.Methods From May 2020 to May 2021,50 pregnant women with fetal growth restriction who were treated in Heze Maternal and Child Health Care Hospital,and 50 normal pregnant women who underwent routine pregnancy examination during the same period were selected as the study subjects,as the study group and the control group,respectively.The levels of vascularization index(VI),flow index(FI),vascularization flow index(VFI),unconjugated estriol(uE3),alpha-fetoprotein(AFP) and beta-human chorionic gonadotropin(β-hCG) biomarkers were compared between the two groups.The receiver operating characteristic(ROC) curve was used to evaluate the diagnostic efficacy of fourdimensional color Doppler ultrasound combined with Down's screening for fetal growth restriction.Results The Doppler ultrasound parameters VI,FI and VFI of the group were 14.39±2.25,20.38±2.15 and 4.08±0.63,respectively,which were lower than 28.55±4.17,32.62±1.77 and 8.25±1.40 of the control group,and the differences were statistically significant(t=21.131,31.079,20.450,all P<0.05).The levels of uE3,AFP and β-hCG were higher than those in the control group,and the differences were statistically significant(all P<0.05).ROC curve analysis showed that the sensitivity and specificity of four-dimensional color Doppler ultrasound combined with Down's screening were better.Conclusion Four-dimensional color Doppler ultrasound combined with Down's screening provide more accurate and comprehensive clinical information for early detection and evaluation of fetal growth restriction,and has high diagnostic efficiency.
