超声斑点追踪显像技术评价甲状腺功能亢进患者左心房功能的价值分析

摘　　要：目的 探讨超声斑点追踪显像(speckle tracking imaging,STI)对甲状腺功能亢进(hyperthyroidism,HT)患者左心房功能的评价效果。方法 回顾性选取2022年8月—2023年7月济南市章丘区人民医院收治的62例HT患者的临床资料,根据诊断结果将其分为亚临床HT组(1级)30例、临床HT组(2级)32例。两组患者均接受STI检查,比较两组左心房功能与容积相关参数、左心房应变参数。结果 临床HT组的左心房主动射血分数为(26.02±3.54)%、被动射血分数为(30.02±3.75)%,均低于亚临床HT组的(30.02±3.46)%、(34.06±3.56)%,差异有统计学意义(t=4.495、4.344,P均<0.05)。临床HT组左心房收缩前容积、最小容积、最大容积均大于亚临床HT组,差异有统计学意义(P均<0.05)。临床HT组左心房舒张晚期峰值应变率、舒张早期峰值应变率均低于亚临床HT组,收缩期峰值应变率高于亚临床HT组,差异有统计学意义(P均<0.05)。结论 STI可以客观评估不同分级HT患者的左心房功能,为临床精准诊疗提供了有力保障。Objective To investigate the evaluation effect of ultrasound speckle tracking imaging(STI) on left atrial function in patients with hyperthyroidism(HT).Methods The clinical data of 62 patients with HT admitted to Zhangqiu District People's Hospital from August 2022 to July 2023 were retrospectively selected.According to the diagnostic results,they were divided into subclinical HT group(grade 1) 30 cases and clinical HT group(grade 2) 32 cases.Both groups received STI examination,and the left atrial function and volume-related parameters and left atrial strain parameters were compared between the two groups.Results The left atrial active ejection fraction was(26.02±3.54)% and passive ejection fraction was(30.02±3.75)% in the clinical HT group,which were lower than(30.02±3.46)% and(34.06±3.56)% in the subclinical HT group,the differences were statistically significant(t=4.495,4.344,both P<0.05).The pre-systolic volume,minimum volume and maximum volume of left atrial in the clinical HT group were bigger than those in the subclinical HT group,and the differences were statistically significant(all P<0.05).The late diastolic peak strain rate and early diastolic peak strain rate of the left atrium in the clinical HT group were lower than those in the subclinical HT group,and the systolic peak strain rate were higher than those in the subclinical HT group,the differences were statistically significant(all P<0.05).Conclusion STI can objectively evaluate the left atrial function of patients with different grades of HT,which provides a strong guarantee for accurate clinical diagnosis and treatment.
