经阴道彩色多普勒超声诊断剖宫产切口瘢痕妊娠的价值分析

摘　　要：目的 探讨对剖宫产术后子宫瘢痕妊娠(cesarean scar pregnancy,CSP)孕妇给予经阴道超声(transvaginal ultrasonography,TVUS)诊断的临床效果。方法 选取2023年2月—2024年2月贵航贵阳医院收治的70例疑似CSP孕妇为研究对象,对全部孕妇实施TVUS、经腹部多普勒超声(transabdominal ultrasound,TAUS)检查,以临床综合诊断结果为金标准,分析诊断效能。结果 70例疑似CSP孕妇中,经临床综合诊断确诊阳性63例,阴性7例,经TAUS诊断阳性56例,阴性14例;经TVUS诊断阳性62例,阴性8例;TVUS诊断的灵敏度为96.83%(61/63)、特异度为85.71%(6/7)、准确度为95.71%(67/70),均高于TAUS诊断的80.95%(51/63)、28.57%(2/7)、75.71%(53/70),差异有统计学意义(χ^(2)=9.277、4.667、10.390,P均<0.05)。结论 应用TVUS诊断CSP的准确度、特异度和灵敏度均较高,可为临床诊断提供可靠依据。Objective To investigate the clinical effect of transvaginal ultrasonography(TVUS) in the diagnosis of cesarean scar pregnancy(CSP).Methods From February 2023 to February 2024,70 pregnant women with suspected CSP admitted to Guihang Guiyang Hospital were selected as the research objects.All pregnant women were examined by TVUS and transabdominal ultrasound(TAUS).The results of clinical comprehensive diagnosis were taken as the gold standard,and the diagnostic efficacy was analyzed.Results Among 70 cases of suspected CSP pregnant women,63 cases were positive and 7 cases were negative by clinical comprehensive diagnosis,56 cases were positive and 14 cases were negative by TAUS diagnosis.TVUS diagnosis confirmed positive in 62 cases,negative in 8 cases;the sensitivity,specificity and accuracy of TVUS diagnosis were 96.83%(61/63),85.71%(6/7) and 95.71%(67/70),respectively,which were higher than those of TAUS diagnosis 80.95%(51/63),28.57%(2/7) and 75.71%(53/70),and the differences were statistically significant(χ~2= 9.277,4.667,10.390,all P<0.05).Conclusion The accuracy,specificity and sensitivity of TVUS in the diagnosis of CSP are high,which can provide a reliable basis for clinical diagnosis.
