摘 要:目的 探讨多层螺旋CT(multi-slice spiral CT,MSCT)三维重建联合磁共振成像(magnetic resonance imaging,MRI)技术对提高腕关节损伤诊断准确度的临床价值。方法 选取2021年3月—2024年3月新疆生产建设兵团第四师医院收治的99例疑似腕关节损伤患者,均接受MRI与MSCT三维重建诊断,并以临床综合诊断为金标准,评估MSCT三维重建及MRI单独及联合检查的诊断结果、诊断效能及诊断符合率。结果 经临床综合诊断检出阳性61例,阴性38例。MSCT三维重建及MRI联合检查的灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于单一检查,差异均有统计学意义(P均<0.05)。联合检查对腕关节损伤的诊断符合率为95.08%(58/61),高于MSCT三维重建的83.61%(51/61)及MRI的78.69%(48/61),差异有统计学意义(χ^(2)=6.452,P<0.05)。结论 MSCT三维重建联合MRI技术能够显著提高腕关节损伤的诊断准确度,相较于单独使用MSCT三维重建或MRI,联合检查在腕关节损伤的诊断中表现出更优越的诊断效能和诊断符合率。Objective To explore the clinical value of multi-slice spiral CT(MSCT) three-dimensional reconstruction combined with magnetic resonance imaging(MRI) in improving the diagnostic accuracy of wrist joint injury.Methods A total of 99 patients with suspected wrist joint injury admitted to the Fourth Division Hospital of Xinjiang Production and Construction Corps from March 2021 to March 2024 were selected.All patients underwent MRI and MSCT three-dimensional reconstruction diagnosis,and clinical comprehensive diagnosis was used as the gold standard.The diagnostic results,diagnostic efficacy and diagnostic coincidence rate of MSCT three-dimensional reconstruction and MRI alone and combined examination were evaluated.Results The results of clinical comprehensive diagnosis were positive in 61 cases and negative in 38 cases.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combined examination of MSCT three-dimensional reconstruction and MRI were higher than those of single examination,and the differences were statistically significant(all P<0.05).The diagnostic coincidence rate of combined examination for wrist joint injury was 95.08%(58/61),which was higher than 83.61%(51/61) of MSCT three-dimensional reconstruction and 78.69%(48/61) of MRI,and the difference was statistically significant(χ~2=6.452,P<0.05).Conclusion MSCT three-dimensional reconstruction combined with MRI can significantly improve the diagnostic accuracy of wrist joint injury.Compared with MSCT three-dimensional reconstruction or MRI alone,combined examination shows superior diagnostic efficiency and diagnostic coincidence rate in the diagnosis of wrist joint injury.
