彩色多普勒超声筛查在诊断胎儿体表畸形中的应用研究

摘　　要：目的 分析在胎儿体表畸形诊断中运用彩色多普勒超声筛查法的价值。方法 选择2023年1月—2024年1月赤峰松山医院收治的80例产前筛查产妇为研究对象,均给予彩色多普勒超声筛查,将临床综合诊断作为金标准,分析彩色多普勒超声的诊断结果、诊断效能及诊断符合率。结果临床综合诊断结果显示阳性8例、阴性72例。彩色多普勒超声筛查结果显示阳性7例、阴性73例,灵敏度为75.00%(6/8),特异度为98.61%(71/72),准确度为96.25%(77/80),阳性预测值为85.71%(6/7),阴性预测值为97.26%(71/73),kappa值为0.779。临床综合诊断结果显示4例肢体异常、2例心脏畸形、1例腹壁裂、1例脊柱异常,彩色多普勒超声筛查对肢体异常、心脏畸形、腹壁裂、脊柱异常的诊断符合率分别为100.00%(4/4)、100.00%(2/2)、0、0。结论 在胎儿体表畸形诊断中,运用彩色多普勒超声筛查的诊断效能较高。Objective To analyze the value of color Doppler ultrasound screening in the diagnosis of fetal surface malformations.Methods From January 2023 to January 2024,a total of 80 pregnant women who underwent prenatal screening in Chifeng Songshan Hospital were selected as the research objects.All of them were given color Doppler ultrasound screening.The clinical comprehensive diagnosis was taken as the gold standard,and the diagnostic results,diagnostic efficacy and diagnostic accuracy rate of color Doppler ultrasound were analyzed.Results The results of clinical comprehensive diagnosis showed that 8 cases were positive and 72cases were negative.The results of color Doppler ultrasound screening showed that 7 cases were positive and 73cases were negative.The sensitivity was 75.00%(6/8),the specificity was 98.61%(71/72),the accuracy was 96.25%(77/80),the positive predictive value was 85.71%(6/7),the negative predictive value was 97.26%(71/73),and the kappa value was 0.779.The results of clinical comprehensive diagnosis showed that there were 4cases of limb abnormality,2 cases of cardiac malformation,1 case of abdominal wall fissure and 1 case of spinal abnormality.The diagnostic coincidence rates of color Doppler ultrasound screening for limb abnormality,cardiac malformation,abdominal wall fissure and spinal abnormality were 100.00%(4/4),100.00%(2/2),0 and 0,respectively.ConclusionI n the diagnosis of fetal surface malformations,the diagnostic efficiency of color Doppler ultrasound screening is high.
