精索静脉曲张中西医结合诊断治疗指南

摘　　要：精索静脉曲张与男性不育的关系、青少年精索静脉曲张是临床广泛关注的两个焦点。目前,国内对于精索静脉曲张的中西医结合诊疗缺乏共识指南,为了制定精索静脉曲张的中西医结合诊疗策略,本编写组根据国内外最新的循证医学资料,结合全国多家单位泌尿外科、男科的实践经验,由中国性学会中医性学分会、中国性学会性医学分会组织相关临床专家,经过起草、学会审核、专家访谈、审稿会等程序充分研究讨论形成本指南。本指南基于循证医学证据、专家临床经验等形成了精索静脉曲张的中西医结合诊疗规范。精索静脉曲张的治疗以缓解阴囊疼痛、改善精液质量、保护患者生育能力为主要目的,应综合考虑患者的年龄、病情、生育需求、既往治疗史及药物不良反应等制定治疗方案,可采用中医或中西医结合的保守治疗进行探索,或采用手术治疗。中医辨证论治基于阴阳辨证将本病证型分为阳证、阴证两类,可采用桂枝茯苓丸、橘核丸等治疗。The relationship between varicocele and male infertility,as well as adolescent varicocele are two focuses of widespread clinical attention.At present,there is a lack of consensus guidelines for the diagnosis and treatment of varicocele by integrated traditional Chinese and western medicine in China.In order to formulate the diagnosis and treatment strategy of varicocele by integrated traditional Chinese and western medicine,according to the latest evidence-based medical data at home and abroad,combined with the practical experience of urology and andrology in many units across the country,the writing team organized relevant clinical experts by guideline writing group of traditional Chinese medicine sexology branch of China Sexology Association,guideline writing group of sexual medicine branch of China Sexology Association.After drafting,academic review,expert interviews,review meetings and other procedures,the diagnosis and treatment guidelines were fully studied and discussed.This guideline is based on the comprehensive evaluation of evidence-based medical evidence and expert clinical experience to formulate the diagnosis and treatment norms of integrated traditional Chinese and western medicine for varicocele.The main purpose of the treatment of varicocele is to relieve scrotal pain,improve semen quality and protect the fertility of patients.When selecting the treatment plan,the patient′s age,condition,fertility needs,previous treatment history and adverse drug reactions are comprehensively considered.Conservative treatment of traditional Chinese medicine or integrated traditional Chinese and western medicine can be used to explore,or surgical treatment can be adapted.Based on Yin and Yang syndrome differentiation,traditional Chinese medicine syndrome differentiation and treatment divides the syndrome types of this disease into two types:Yang syndrome and Yin syndrome.which can be treated with Guizhi Fuling Pill and Juhe Pill.
