摘 要:目的探讨精子DNA碎片率(DFI)与精液参数的相关性及其在男性生育力评估中的应用。方法选取2021年4月至2024年4月河北北方学院附属第一医院生殖医学科收治的1345例男性患者作为研究对象,根据精子DFI数值大小分为DFI<15%组(n=815)、15%≤DFI<30%组(n=389)和DFI≥30%组(n=141)。比较三组精液参数,采用Spearman法分析精子DFI与各项精液参数的相关性,根据世界卫生组织(WHO)规定的参考值对各参数进行分组比较,采用受试者工作特征(ROC)曲线评价精子DFI对少弱精子症的诊断价值。结果15%≤DFI<30%组和DFI≥30%组精液浓度、精子总数、精子活动率和前向运动精子率(PR)均低于DFI<15%组,年龄和精子DFI高于DFI<15%组(P<0.05)。精子DFI与年龄、精液体积呈正相关(P<0.05),与精液浓度、精子总数、精子活动率、PR呈负相关(P<0.001)。对于精液浓度、精子总数、精子活动率和PR,参数异常组精子DFI均高于参数正常组(P<0.05)。弱精子症组和少弱精子症组精子DFI均高于正常组和少精子症组(P<0.001);精子DFI对少精子症、弱精子症和少弱精子症均具有较好的辅助诊断效能,对弱精子症和少弱精子症的预测效能更显著(P<0.001)。结论精子DFI是评价精液质量和精子功能的重要指标,其联合精液常规检测可更全面准确地评估男性生育力。Objective To investigate the correlation of sperm DNA fragmentation index(DFI)with semen parameters and its application in male fertility assessment.Methods A total of 1,345 male patients admitted to the Department of Reproductive Medicine of the First Affiliated Hospital of Hebei North University from April 2021 to April 2024 were selected as study subjects.The patients were divided into group DFI<15%(n=815),group 15%≤DFI<30%(n=389)and group DFI≥30%(n=141)according to DFI value,and semen parameters among the three groups were compared.Spearman method was used to analyze the correlation between sperm DFI and semen parameters.The parameters were compared according to the reference values defined by the World Health Organization(WHO),and receiver operating characteristic(ROC)curve was used to evaluate the diagnostic efficacy of sperm DFI for oligoasthenospermia.Results Sperm concentration,total sperm count,sperm motility rate and progressive motility rate(PR)in group 15%≤DFI<30%and group DFI≥30%were all significantly lower compared with those in group DFI<15%,whereas age and sperm DFI were significantly higher(P<0.05).Sperm DFI was positively correlated with age and semen volume(P<0.05),but significantly negatively correlated with sperm concentration,total sperm count,sperm motility rate and PR(P<0.001).Sperm DFI of the groups with abnormal semen parameters was higher than that of the groups with normal semen parameters for sperm concentration,total sperm count,sperm motility rate and PR(P<0.05).Sperm DFI levels in asthenospermia group and oligoasthenospermia group were significantly higher than those in normal group and oligospermia group(P<0.001).Sperm DFI had good diagnostic efficiency for oligospermia,asthenospermia and oligoasthenospermia,and had more significant predictive efficacy for asthenospermia and oligoasthenospermia(P<0.001).Conclusions Sperm DFI was an important index to evaluate semen quality and sperm function,and its combination with routine semen analysis can assess male fertility more co
