慢性前列腺炎患者尿道分泌物病原体检测结果及其与前列腺液白细胞和NIH-CPSI评分相关性的研究

摘　　要：目的探讨慢性前列腺炎(CP)患者尿道分泌物沙眼衣原体(Ct)、解脲支原体(Uu)、人型支原体(Mh)、淋病奈瑟球菌(Ng)检测结果及其与前列腺液白细胞和美国国立卫生研究院慢性前列腺炎症状指数(NIH-CPSI)评分的相关性。方法回顾性分析2022年3月至2023年8月联勤保障部队第九四○医院收治的166例CP患者的临床资料,收集患者尿道分泌物Ct、Uu、Mh、Ng检测结果,分析患者各病原体感染情况与前列腺液白细胞、NIH-CPSI总评分、NIH-CPSI疼痛及排尿症状评分的相关性。结果Ct阳性率为2.41%(4/166),Uu阳性率为25.30%(42/166),Mh阳性率为9.64%(16/166),Ng阳性率为1.81%(3/166)。Ct、Uu、Mh感染情况与前列腺液白细胞无相关性(P>0.05);Ng感染与前列腺液白细胞呈正相关(P<0.05)。Ct、Uu、Mh、Ng感染情况与NIH-CPSI总评分、疼痛及排尿症状评分均无相关性(P>0.05)。结论CP患者尿道分泌物Uu和Mh阳性率较高,尿道分泌物病原体与前列腺液白细胞和NIH-CPSI评分相关性较低,在临床诊疗中应结合其他手段评估CP患者病情和疗效。Objective To explore the detection of chlamydia trachomatis(Ct),ureaplasma urealyticum(Uu),mycoplasma hominis(Mh),and neisseria gonorrhoeae(Ng)in the urethral secretion of patients with chronic prostatitis(CP),and their correlation with the white blood cells in prostatic fluid and the National Institutes of Health-chronic prostatitis symptom index(NIH-CPSI).Methods A retrospective analysis was conducted on the clinical data of 166 CP patients admitted to the 940th Hospital of Joint Logistics Support Force of Chinese PLA from March 2022 to August 2023.The Ct,Uu,Mh,and Ng test results of the urethral secretion from patients were collected,and the correlation between the infection status of various pathogens in patients and the prostate fluid white blood cells,NIH-CPSI total score,NIH-CPSI pain and urinary symptom score was analyzed.Results The Ct positivity rate was 2.41%(4/166),the Uu positivity rate was 25.30%(42/166),the Mh positivity rate was 9.64%(16/166),and the Ng positivity rate was 1.81%(3/166).There was no correlation between the Ct,Uu,Mh infection and the white blood cells in prostate fluid(P>0.05).The Ng infection was positively correlated with the white blood cells in prostate fluid(P<0.05).There was no correlation between the Ct,Uu,Mh,Ng infection status and NIH-CPSI total score,pain and urinary symptom score(P>0.05).Conclusions The positive rates of Uu and Mh in the urethral secretion of CP patients are relatively high,and the correlation between the pathogen detection in urethral secretion and the white blood cells in prostate fluid and NIH-CPSI score is low.In clinical diagnosis and treatment,other methods should be combined to evaluate the condition and efficacy of CP patients.
