中重度子宫内膜异位症患者凝血功能指标与盆腔粘连和疼痛程度的相关性

摘　　要：目的探讨中重度子宫内膜异位症(EMT)患者凝血功能指标与盆腔粘连和疼痛程度的关系。方法选取2021年3月至2024年3月石家庄市人民医院收治的106例EMT患者作为研究对象,根据美国生殖医学学会分期评分表评分分为中重度组(n=68)和轻度组(n=38)。比较两组凝血功能指标、盆腔粘连评分、视觉模拟评分法(VAS)评分,采用Spearman法分析凝血功能指标与盆腔粘连和疼痛程度的相关性。结果中重度组活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、凝血酶原时间(PT)均短于轻度组,纤维蛋白原(FIB)和D-二聚体(DD)水平均高于轻度组(P<0.05)。中重度组盆腔粘连各项评分和总分均高于轻度组(P<0.05)。中重度组VAS评分高于轻度组(P<0.05)。APTT、TT、PT与盆腔粘连和VAS评分均呈负相关,FIB、DD与盆腔粘连和VAS评分均呈正相关(P<0.05);盆腔粘连与VAS评分呈正相关(r=0.371,P<0.05)。结论中重度EMT患者凝血功能指标与盆腔粘连和疼痛程度具有相关性。Objective To explore the relationship between coagulation function indexes and pelvic adhesion,pain degree in patients with moderate to severe endometriosis(EMT).Methods A total of 106 patients with EMT admitted to Shijiazhuang People′s Hospital were enrolled as the research objects between March 2021 and March 2024.According to scores of American society for reproductive medicine(ASRM),they were divided into moderate-severe group(n=68)and mild group(n=38).The coagulation function indexes,scores of pelvic adhesion and visual analogue scale(VAS)in the two groups were compared.The correlation between coagulation function indexes and pelvic adhesion,pain degree was analyzed by Spearman method.Results The activated partial thromboplastin time(APTT),thrombin time(TT)and prothrombin time(PT)in moderate-severe group were all shorter than those in mild group,levels of fibrinogen(FIB)and D-dimer(DD)were both higher than those in mild group(P<0.05).All scores and total score of pelvic adhesion in moderate-severe group were both higher than those in mild group(P<0.05),and VAS score was also higher than that in mild group(P<0.05).APTT,TT and PT were negatively correlated with pelvic adhesion and VAS score,while FIB and DD were positively correlated with pelvic adhesion and VAS score(P<0.05).The pelvic adhesion was positively correlated with VAS score(r=0.371,P<0.05).Conclusions The coagulation function indexes are related to pelvic adhesion and pain degree in patients with moderate to severe EMT.
