桂枝茯苓胶囊辅助治疗子宫内膜息肉的效果及机制分析

摘　　要：目的探讨桂枝茯苓胶囊辅助治疗子宫内膜息肉(EP)的临床效果,并初步分析其作用机制。方法选取2021年1月至2023年3月南京同仁医院收治的100例采用手术治疗的EP患者作为研究对象,随机分为对照组与观察组,各50例。对照组术后予以地屈孕酮片治疗,观察组在对照组的基础上予以桂枝茯苓胶囊治疗。比较两组疗效,比较两组治疗前后中医证候积分、子宫内膜厚度、炎症因子、雌激素受体(ER)和孕激素受体(PR)水平及术后6、12个月的复发率。结果观察组临床疗效显著优于对照组(P<0.05)。治疗后,两组中医证候积分、子宫内膜厚度均较治疗前显著改善,且观察组优于对照组(P<0.05)。治疗后,对照组间质PR水平较治疗前降低,观察组腺体与间质ER、PR水平均较治疗前降低且均低于对照组(P<0.05)。治疗后,两组白介素-6、白介素-8、C反应蛋白、肿瘤坏死因子-α水平均较治疗前降低,且观察组低于对照组(P<0.05)。观察组术后6、12个月复发率均显著低于对照组(P<0.05)。结论EP术后应用桂枝茯苓胶囊辅助治疗可有效提高疗效,改善中医证候积分、子宫内膜厚度,降低复发风险,其机制可能与调节性激素水平及减轻炎症反应有关。Objective To investigate the clinical effect of Guizhi Fuling Capsule in the treatment of endometrial polyp(EP)and to preliminarily analyze its mechanism of action.Methods A total of 100 EP patients treated with surgery in Nanjing Tongren Hospital from January 2021 to March 2023 were selected as the study objects and randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with desdrogesterone tablets after surgery,and the observation group was treated with Guizhi Fuling Capsule on the basis of the control group.The efficacy of the two groups was compared,and the traditional Chinese medicine(TCM)syndrome score,endometrial thickness,inflammatory factors,estrogen receptor(ER),progesterone receptor(PR)levels before and after treatment,as well as the recurrence rate of 6 and 12 months after surgery were compared.Results The clinical effect of the observation group was significantly better than that of the control group(P<0.05).After treatment,TCM syndrome score and endometrial thickness in both groups were significantly improved compared with before treatment,and observation group was better than control group(P<0.05).After treatment,the level of interstitial PR in control group was lower than before treatment,and the levels of ER and PR in glandular and interstitial tissues in observation group were lower than before treatment(P<0.05).After treatment,the levels of interleukin-6,interleukin-8,C-reactive protein and tumor necrosis factor-αin both groups were lower than before treatment,and the observation group was lower than the control group(P<0.05).The recurrence rates of the observation group were significantly lower than those of the control group at 6 and 12 months after surgery(P<0.05).Conclusions The adjuvant treatment of Guizhi Fuling Capsule can effectively improve the curative effect,improve the TCM syndrome score,endometrial thickness,and reduce the risk of recurrence after EP surgery.The mechanism may be related to the regulation of sex hormone l
