苍附导痰汤联合地屈孕酮治疗多囊卵巢综合征不孕患者的疗效及对性激素水平的影响

摘　　要：目的探讨苍附导痰汤联合地屈孕酮对多囊卵巢综合征(PCOS)不孕患者的疗效及对性激素水平的影响。方法选取2021年1月至2023年1月青岛市第五人民医院收治的86例PCOS患者作为研究对象,采用随机数字表法分为对照组和试验组,各43例。对照组仅口服地屈孕酮治疗,试验组在地屈孕酮的基础上联合苍附导痰汤治疗。比较两组的临床疗效、治疗前后卵巢情况和性激素水平,以及随访期间妊娠率、排卵率和不良反应情况。结果治疗后,试验组治疗总有效率显著高于对照组(P<0.05)。治疗后两组卵巢体积、子宫内膜厚度,以及血清卵泡刺激素(FSH)、睾酮(T)、黄体生成素(LH)水平均显著降低,而血清雌二醇(E_(2))水平显著升高,且试验组较对照组改善更显著(P<0.05)。随访期间,试验组妊娠率和排卵率显著高于对照组、药物不良反应发生率显著低于对照组(P<0.05)。结论苍附导痰汤联合地屈孕酮能够显著改善PCOS患者的卵巢功能和性激素水平,进而提升排卵率和妊娠率。Objective To explore the therapeutic effect of Cangfu Daotan Decoction combined with dydrogesterone on infertile patients with polycystic ovary syndrome(PCOS)and its impact on sex hormone levels.Methods A total of 86 PCOS patients who accepted by Qingdao Fifth People′s Hospital from January 2021 to January 2023 were regarded as study objects,and were randomly assigned to either a control group or an experimental group by random number table method,with 43 cases in each group.The control group was only treated with oral dydrogesterone,while the experimental group was treated with Cangfu Daotan Decoction on the basis of dydrogesterone.The two groups were compared in terms of the therapeutic effects,the ovarian condition and sex hormone levels before and after treatments,as well as the pregnancy rate,ovulation rate and adverse reaction condition during follow-up.Results After treatment,the total effective rate of the experimental group was higher than that in the control group(P<0.05).After treatment,the volume of ovaries,endometrial thickness,as well as serum follicle stimulating hormone(FSH),testosterone(T),and luteinizing hormone(LH)levels in the two groups were significantly decreased,while serum estradiol(E_(2))level was significantly increased,and the experimental group showed more significant improvement than the control group(P<0.05).During the follow-up period,the pregnancy rate and ovulation rate of the experimental group were higher than those of the control group(P<0.05),and the incidence of adverse drug reactions in the experimental group was lower than that of the control group(P<0.05).Conclusions The combination of Cangfu Daotan Decoction and dydrogesterone can significantly improve the ovarian function and sex hormone levels in PCOS patients,and therefore increase ovulation rate and pregnancy rate.
