摘  要:目的:通过检测多囊卵巢综合征(PCOS)患者血清中血管生成素样蛋白2(ANGPTL2)及巨噬细胞炎症蛋白-1α(MIP-1α)的表达水平,探讨PCOS与慢性低度炎症的关系。方法:选取2022年7月至10月就诊于河北大学附属医院生殖医学科的PCOS患者120例和正常女性40例。酶联免疫法(ELISA)检测血清ANGPTL2、MIP-1α表达水平,化学发光法检测血清中卵泡刺激素(FSH)、黄体生成素(LH)、睾酮(T)、雌二醇(E 2)、孕激素(P)、泌乳素(PRL)水平;对所有研究对象进行胰岛素释放实验(IRT)及葡萄糖耐量实验(OGTT)。按T、身体质量指数(BMI)及胰岛素抵抗指数(HOMA-IR)将PCOS患者分成8个亚组:PCOS非高T非肥胖非IR、PCOS-IR肥胖非高T、PCOS-IR非高T非肥胖、PCOS-肥胖非高T非IR、PCOS-肥胖高T非IR、PCOS-IR高T非肥胖、PCOS高T非肥胖非IR以及PCOS-IR高T肥胖。结果:正常对照组与PCOS非高T非肥胖非IR组比较,血清ANGPTL2、MIP-1a水平明显偏低(P<0.05)。PCOS-IR非高T非肥胖组与PCOS非高T非肥胖非IR组比较,ANGPTL2、MIP-1a水平明显升高(P<0.05)。PCOS肥胖非高T非IR组与PCOS非高T非肥胖非IR组比较,ANGPTL2、MIP-1a水平亦明显升高(P<0.05)。回归分析结果显示,所有研究对象中ANGPTL2及MIP-1α均与HOMA-IR、BMI、T及LH/FSH均表现为正相关(P<0.05)。PCOS组中,ANGPTL2及MIP-1α与BMI、HOMA-IR呈正相关(P<0.05)。结论:慢性低度炎症在PCOS发病机制中起着重要作用,ANGPTL2、MIP-1α可作为诊断PCOS及评估其严重程度的新的生物标志物,为治疗PCOS提供了新方向。Objective:To investigate the relationship between PCOS and chronic low-grade inflammation,and to speculate the pathogenesis of PCOS,by detecting the expression of ANGPTL2 and MIP-1αin the serum of patients with PCOS,so as to provide a new feasible basis for the PCOS.Methods:120 patients diagnosed with PCOS who were admitted to the Reproductive Center outpatient Clinic of the Affiliated Hospital of Hebei University from July to October 2022 were selected as the experimental group(PCOS group),and 40 women diagnosed with male factor infertility during the same period and whose other laboratory examination results were normal were collected as the normal control group.Serum levels of ANGPTL2 and MIP-1αwere detected by enzyme-linked immunoassay,follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E 2),progesterone(P)and prolactin(PRL)ande testosterone(T)levels in serum were detected by chemiluminescence method.Oral Glucose Tolerance Test(OGTT)and Insulin Release Test(IRT)were performed for all enrolled subjects.PCOS were divided into 8 subgroups according to T,body mass index(BMI)and insulin resistance index(HOMA-IR),as follows:PCOS non-high T non-obese non-IR,PCOS-IR non-high T non-obesity,PCOS-IR non-high T non-obesity,PCOS-obesity non-high T non-IR,PCOS-obesity non-IR,PCOS-IR high-T non-obesity,PCOS high-T non-IR,PCOS high-T non-obesity,PCOS high-T non-IR,PCOS high-T non-obesity,PCOS high-T non-IR,and PCOS-IR high-T non-obesity.Results:Serum levels of ANGPTL2 and MIP-1αwere significantly lower in the normal control group compared to the PCOS non-high testosterone,non-obese,and non-insulin resistant group(P<0.05).The concentrations of ANGPTL2 and MIP-1αin the PCOS insulin-resistant,non-high testosterone,non-obese group were markedly elevated relative to those in the PCOS non-high testosterone,non-obese,insulin-sensitive cohort(P<0.05).Furthermore,ANGPTL2 and MIP-1αlevels were significantly increased in the PCOS obese,insulin-sensitive group when contrasted with the PCOS non-high testosterone,insu
