筋膜刀干预对赛艇运动员赛前大负荷训练后免疫、炎症和氧化应激水平的影响

摘  要:目的:探讨筋膜刀干预对赛艇运动员大负荷训练后疼痛、主观疲劳、免疫机能、炎症反应和氧化应激的影响。方法:以26名优秀男子赛艇运动员为研究对象,随机分为筋膜刀干预组(以下简称“干预组”)和对照组。干预组运动员于赛前训练阶段实施每周3次、每次30~40 min、共2周的筋膜刀干预治疗;对照组安静休息。分别在干预前和干预后填写疼痛量表和疲劳量表,并空腹肘静脉采血测量免疫机能、炎症和氧化应激水平等相关指标。结果:1)与干预前比较,干预组干预后疼痛量表评分有下降趋势,疲劳量表评分显著下降(P<0.05),而对照组疼痛量表和简易疲劳量表评分无显著变化;两组间疲劳量表评分的变化具有显著差异(-20.00%vs 5.61%,P=0.02)。2)干预后,两组别循环血肌酐水平无显著变化,肌酸激酶水平均显著升高(P<0.05);两组间肌酐、肌酸激酶水平的变化无显著差异。3)与干预前比较,两组别循环血白细胞计数、单核细胞和淋巴细胞百分比在干预后均未发生显著变化,对照组中性粒细胞百分比有显著升高的趋势;两组间白细胞计数、中性粒细胞和淋巴细胞百分比的变化无显著差异,单核细胞百分比的变化有显著差异的趋势。4)干预后,两组别B淋巴细胞百分比、CD8^(+)T淋巴细胞百分比均显著升高(P<0.05),T淋巴细胞百分比、CD4^(+)T淋巴细胞百分比仅对照组显著升高(P<0.05),且对照组NK淋巴细胞百分比有下降的趋势;两组组间T淋巴细胞百分比、B淋巴细胞百分比、NK淋巴细胞百分比以及CD4^(+)、CD8^(+)T淋巴细胞百分比的变化均无显著差异。5)干预后,对照组和干预组循环血白细胞介素-6、肿瘤坏死因子-α、活性氧、超氧化物歧化酶水平均显著下降(P<0.05),C反应蛋白、丙二醛未发生显著改变,上述指标的变化两组组间均无显著性差异。结论:持续2周的筋膜刀干预有助于缓解赛前大负荷训�Objective:This study aims to investigate the effects of myofascial release using fascial knife on pain,fatigue,immune function,inflammatory response,and oxidative stress in rowers after heavy-load training before competition.Methods:Twenty-six well-trained male rowers were randomly assigned into either a myofascial intervention group(INT)or a control group(CON).The INT group received myofascial treatment three times a week for 30-40 minutes over two weeks,while the CON group remain rest.Before and after the intervention,pain and fatigue levels were assessed using scales.Additionally,fasting blood samples were collected to measure biomarkers related to immunity,inflammation,and oxidative stress.Results:1)After the intervention,the INT group showed a significant decrease in both pain scale scores and fatigue scale scores(P<0.05),while the CON group showed no significant changes in either scores.A significant difference in the change in fatigue scale scores were observed between the two groups(-20.00%vs 5.61%,P=0.02).2)While circulating creatinine levels did not change in either the INT or CON groups,circulating creatine kinase levels significantly increased in both groups(P<0.05)after the intervention.There were no significant difference in the changes of creati‐nine and creatine kinase between the two groups.3)Compared with baseline value,the circulat‐ing white blood cell counts,monocyte percentage,and lymphocyte percentage did not show sig‐nificant changes in either group after intervention.The CON group showed a tendency for a sig‐nificant increase in neutrophil percentage.No significant differences were observed between the groups regarding changes in white blood cell count,neutrophil percentage,and lymphocyte per‐centage.However,there was a tendency indicating a significant difference in the change of monocyte percentage.4)After the intervention,both groups showed a significant increase in the percentages of B lymphocytes and CD8^(+)T lymphocytes(P<0.05).The percentages of T lym‐phocytes and CD4^(+
