摘  要:目的:探讨运动联合连续性θ爆发式磁刺激(continuous theta burst stimulation,cTBS)干预改善孤独症谱系障碍(以下简称“孤独症”)儿童核心症状的协同效应。方法:54名3~10岁孤独症儿童分为cTBS干预组(cTBS group,TG)、运动干预组(exercise group,EG)、运动联合cTBS干预组(exercise+cTBS group,ETG)和对照组(control group,CG)。TG接受cTBS刺激(5天/周,3次/天,12周),EG接受组合球类运动干预(ball combination training program,BCTP)(5天/周,45 min/天,12周),ETG接受组合球类联合cTBS干预,CG接受常规康复。采用重复刻板行为检查表修订版(repetitive behavior scale-revised edition,RBS-R)和社交反应量表第二版(social responsiveness scale-second edition,SRS-2)评估干预前后核心症状。结果:1)在刻板行为方面,3种干预显著改善了孤独症儿童RBS-R量表总分,在子维度上仅cTBS干预改善了固定行为。相较于单一干预,联合干预没有表现出显著的优势。2)在社交障碍方面,3种干预显著改善了SRS-2量表总分,在子维度上cTBS干预改善了社交知觉、社交沟通、社交动机和固定行为,BCTP干预改善了社交知觉和社交认知,联合干预改善了社交知觉、社交认知、社交沟通以及行为方式。相较于单一干预,联合干预在改善社交认知、社交沟通、行为方式上更有优势。结论:运动干预、cTBS干预以及联合干预均能够改善孤独症儿童的核心症状,且联合干预对社交障碍改善有协同效应。Objective:To investigate the combined synergistic effect of continuous theta burst stimulation(cTBS)and exercise intervention on core symptoms in children with autism spectrum disorders(ASD).Methods:54 children with ASD aged 3 to 10 years old were divided into four groups:cTBS group(TG),exercise intervention group(EG),exercise intervention combined with cTBS group(ETG),and control group(CG).The TG engaged cTBS(5 d/week,3 session/d,12 weeks),the EG engaged ball combination training program(BCTP)(5 d/week,45 min/d,12 weeks),the ETG underwent both treatments and CG engaged routine rehabilitation.The core symptoms before and after intervention was evaluated using the repetitive behavior scale-revised(RBS-R)and the social responsiveness scale-second edition(SRS-2)reported by parents.Results:1)In terms of repetitive behaviors,three interventions significantly improved the RBS-R T-scores,while only cTBS improved fixed behavior in the sub scales.Compared to a single intervention,combined intervention did not show significant advantages.2)In terms of social communication impairment,three interventions significantly improved the total score of the SRS-2 T-scores.In the sub scales,cTBS improved social awareness,social communication,social motivation,and autistic mannerisms,BCTP improved social awareness and social cognition,and combined intervention improved social awareness,social cognition,social communication,and autistic mannerisms.Compared to a single intervention,combined intervention has more advantages in improving social cognition,social communication,and autistic mannerisms.Conclusions:Exercise intervention,cTBS,and combined intervention can effectively improve the core symptoms of children with ASD,and combined intervention has a synergistic effect on improving social communication impairment.
