后组筛房向筛区外扩展的识别及意义

摘　　要：熟知鼻窦相关解剖是安全有效地开展内镜鼻窦手术(ESS)的前提,而鼻窦解剖存在极大的变异,因此鼻科医生在ESS术前均需细致阅片。筛窦的气化变异决定了ESS中各个鼻窦开放的难易程度,因此术前识别筛窦的气化变异尤其重要。后组筛房也具有极大的变异特征,可以向眶上、眶下以及蝶骨体不同区域扩展。我们基于前期研究以及近期文献,将后组筛房扩展的影像解剖做一回顾,主要包括其气化来源、所在位置、毗邻结构和内镜手术学意义等,希望提升鼻科医师对此类变异的认识。The prerequisite for a nasal endoscopic surgeon to safely and effectively perform endoscopic sinus surgery(ESS)is familiarity with the anatomy of the paranasal sinuses.However,there are significant variations in sinus anatomy,so endoscopic sinus surgeons need to carefully review the images before ESS.The pneumatization variation of the ethmoid sinus determines the difficulty of opening the frontal sinus,sphenoid sinus,and maxillary sinus during ESS,so preoperative identification of pneumatization variation of the ethmoid sinus is particularly important.The posterior ethmoidal cell(PE)also has significant variability and can extend toward areas such as the orbital region,infraorbital region,and sphenoid body.Based on previous anatomical studies and recent literature of the PE,this paper summarizes the types of pneumatization extension in the PE,including the origin of the cell,the location of the cell,adjacent structures around the cell,and their significance for ESS,with a view to enhancing the understanding of these variations among rhinologists.
