摘 要:目的探索血清成纤维细胞生长因子19(fibroblast growth factor 19,FGF19)、半胱氨酸天冬氨酸蛋白水解酶-3(cysteinyl aspartate specific proteinase-3,Caspase-3)、γ-谷氨酰转肽酶(glutamate transpeptidase,GGT)水平与胆道闭锁(biliary atresia,BA)患儿肝纤维化的相关性。方法本研究选取2021年3月至2024年3月在河南省儿童医院郑州儿童医院接受治疗的87名BA患儿作为研究对象,将其标记为BA组。同时,选取70名健康婴儿作为对照组。根据病理学检查结果,将BA组患儿划分为明显肝纤维化组(57例)和轻度或无肝纤维化组(30例)。研究收集两组婴儿的年龄和性别信息,并使用酶联免疫吸附法(enzyme linked immunosorbent assay,ELISA)检测血清FGF19、Caspase-3、GGT水平。通过比较两组的肝功能和纤维化指标,运用Pearson相关性分析评估血清FGF19、Caspase-3、GGT水平和肝纤维化指标之间的相关性。此外,本研究还比较不同肝纤维化程度的BA患儿在血清FGF19、Caspase-3、GGT水平上的差异,并采用受试者工作特征(receiver operating characteristic,ROC)曲线分析这些血清标志物对BA患儿明显肝纤维化的诊断价值。结果BA组患儿血清FGF19、Caspase-3、GGT水平高于对照组(P<0.05),BA组天冬氨酸转氨酶(aspartate aminotransferase,AST)、直接胆红素(direct bilirubin,DBIL)、总胆红素(total bilirubin,TBIL)、丙氨酸转氨酶(alanine aminotransferase,ALT)、层黏蛋白(laminin,LN)、IV型胶原(type IV collagen,IV-Col)、Ⅲ型前胶原(type III procollagen,PCⅢ)、透明质酸(hyaluronic acid,HA)水平高于对照组(P<0.05),血小板(platelet,PLT)水平低于对照组(P<0.05),BA组患儿血清FGF19、Caspase-3、GGT水平与LN、IV-Col、PCⅢ、HA及肝纤维化评分正相关(P<0.05),明显肝纤维化组患儿血清FGF19、Caspase-3、GGT水平高于轻度或无肝纤维化组(P<0.05),血清FGF19、Caspase-3、GGT水平联合诊断BA患儿明显肝纤维化的曲线下面积(area under the curve,AUC)高于Objective To explore the correlation between serum levels of fibroblast growth factor 19(FGF19),cysteinyl aspartate specific proteinase-3(Caspase-3),and glutamate transpeptidase(GGT)with the liver fibrosis degree in children with biliary atresia(BA).Methods A total of 87 children with BA children admitted to Zhengzhou Children′s Hospital of Henan Children′s Hospital from March 2021 and March 2024 were selected for the study,designated as the BA group,and 70 healthy infants were selected as the control group during the same period.According to the pathological examination results,the children in BA group were separated into a obvious liver fibrosis group(57 cases)and a mild or no fibrosis group(30 cases).The age and gender information of infants in two groups were collected.Enzyme linked immunosorbent assay(ELISA)was applied to detect serum FGF19,Caspase-3,and GGT levels.The liver function and fibrosis indicators were compared between the two groups.Pearson correlation was applied to analyze the correlation between serum FGF19,Caspase-3,GGT levels and liver fibrosis indicators,and the serum FGF19,Caspase-3,and GGT levels in BA children with different degrees of liver fibrosis was compared.Receiver operating characteristic(ROC)curve was applied to analyze the diagnostic value of serum FGF19,Caspase-3,and GGT levels for obvious liver fibrosis in children with BA.Results The serum levels of FGF19,Caspase-3,and GGT in the BA group were higher than those in the control group(P<0.05),the levels of aspartate aminotransferase(AST),direct bilirubin(DBIL),total bilirubin(TBIL),alanine aminotransferase(ALT),laminin(LN),type IV collagen(IV-Col),type III procollagen(PCIII),and hyaluronic acid(HA)in the BA group were higher than those in the control group(P<0.05),the level of platelet(PLT)was lower than that in the control group(P<0.05),the serum levels of FGF19,Caspase-3,and GGT in the BA group were positively correlated with LN,IV-Col,PCIII,HA,and liver fibrosis score(P<0.05),the serum levels of FGF19,Caspase-3,and GGT in
