超声心动图参数在老年冠心病诊断及冠脉狭窄程度评估中的应用

摘  要:目的探讨超声心动图参数在老年冠心病诊断及冠脉狭窄程度评估中的应用价值。方法在焦作市第二人民医院超声医学科选取100例冠心病患者,将其纳入冠心病组,另收入同期在焦作市第二人民医院接受体检的健康志愿者100例作对照组,对两组开展超声心动图检查,并比较两组超声心动图参数[左室射血分数(left ventricular ejection fraction,LVEF)、左室舒张末期容积(left ventricular end diastolic volume,LVEDV)、左室收缩末期容积(the LV-end-systolic volume,LVESV)、左室舒张末期内径(left ventricular end diastolic diameter,LVEDd)],对冠心患者开展冠状动脉造影,按照Gensini评分标准对造影结果进行评估,根据冠脉狭窄程度将患者分为轻度狭窄组(n=45)、中重度狭窄组(n=55),对两组超声心动图参数进行比较,受试者工作特征(receiver operating characteristic,ROC)曲线用以分析超声心动图各指标诊断冠心病及评估冠状动脉狭窄程度的价值。结果冠心病组LVEF低于对照组,LVEDV、LVESV、LVEDd高于对照组,差异有统计学意义(P<0.05)。经ROC分析,LVEF、LVEDV、LVESV、LVEDd诊断冠心病的曲线下面积分别为0.953、0.967、0.986、0.976,最佳截断值分别为51.495、107.500、39.500、98.500。轻度狭窄组LVEF高于对照组,LVEDV、LVESV、LVEDd低于中重度狭窄组,差异有统计学意义(P<0.05)。LVEF、LVEDV、LVESV、LVEDd评估冠脉中重度狭窄的曲线下面积分别为0.937、0.863、0.978、0.861,最佳截断值分别为47.495、123.500、49.500、110.500。结论超声心动图参数用于老年冠心病诊断及冠脉狭窄程度评估具有重要意义。Objective To explore the application value of echocardiographic parameters in the diagnosis of elderly coronary heart disease and the evaluation of coronary artery stenosis degree.Methods A total of 100 patients with coronary heart disease were selected from the Department of Ultrasound Medicine of the Second People′s Hospital of Jiaozuo City and included into the coronary heart disease group,and another 100 healthy volunteers who underwent physical examination in our hospital during the same period were included as the control group.Echocardiography was performed in the two groups,and echocardiographic parameters[left ventricular ejection fraction(LVEF),left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),and left ventricular end-diastolic diameter(LVEDd)]were compared between the two groups.Coronary angiography was performed on coronary patients,and the angiography results were evaluated according to Gensini scoring criteria.Patients were divided into mild stenosis group(n=45)and moderate and severe stenosis group(n=55)according to the degree of coronary stenosis.Echocardiographic parameters of the two groups were compared.ROC was used to analyze the value of echocardiographic indicators in diagnosing coronary heart disease and evaluating the degree of coronary artery stenosis.Results LVEF in the coronary heart disease group was lower than that in the control group,and LVEDV,LVESV,and LVEDd were higher than those in the control group,with significant differences(P<0.05).Through ROC analysis,the areas under the curve of LVEF,LVEDV,LVESV,and LVEDd in the diagnosis of coronary heart disease were 0.953,0.967,0.986,and 0.976,respectively,and the optimal cut-off values were 51.495,107.500,39.500,and 98.500,respectively.LVEF in the mild stenosis group was higher than that in the control group,and LVEDV,LVESV,and LVEDd were lower than those in the moderate-severe stenosis group,with significant differences(P<0.05).The areas under the curve of LVEF,LVEDV,LVESV,and LVEDd in the evalu
